摘要
目的运用GLIM标准评价广州市越秀区社区老年人营养不良及营养不良风险情况,评估GLIM标准进行营养不良筛查的潜在作用。方法对286名65岁以上社区老年人进行问卷调查、人体测量、体成分测量、血液生化学检查,分析探讨GLIM与其他指标的相关性,采用灵敏度、特异度等指标评价GLIM标准评价结果。结果营养不良组的上臂肌围、腰围、去脂体重(FFM)、去脂体重指数(FFMI)、骨骼肌质量指数(SMI)、总体水(TBW)等指标均显著低于非营养不良组(P<0.05);GLIM评定结果与上臂肌围、腰围、去脂体重(FFM)、去脂体重指数(FFMI)、骨骼肌质量指数(SMI)、总体水(TBW)呈负相关(P<0.05);和单独使用上臂肌围和去脂体重指数筛查营养不良相比,GLIM显示高特异度、较高敏感度。结论GLIM标准评价结果和人体测量学指标、人体组分存在良好的相关性,可较为全面、准确地了解老年人的营养状况,具有作为社区老年人营养不良状况筛查工具的潜力。
Objective To evaluate the malnutrition and the risk of malnutrition among the elderly in Yuexiu District,Guangzhou City by using the Global Leadership Initiative on Malnutrition(GLIM)criteria,and to evaluate the potential role of this criteria in malnutrition screening.Methods 286 community elderly over 65 years old were investigated by questionnaire,and their anthropometrical measurements,body composition measurements and blood biochemical indicators were collected.Spearman correlation analysis was applied to explore the correlation between GLIM and other indicators.Sensitivity,specificity and other indicators were used to evaluate the consistency of GLIM criteria evaluation results.Results The mid-arm circumference(MAC),waist circumference fat-free mass(FFM),fat-free mass index(FFMI),skeletal muscle mass index(SMI),total body water(TBW)and other indicators of the malnourished group were significantly lower than well-nourished group.GLIM evaluation results and MAC,waist circumference,FFM FFMI,SMI,TBW were negatively correlated.Compared with the use of the MAC and FFMI respectively in screening for malnutrition,GLIM criteria showed high specificity and relatively high sensitivity.Conclusion The evaluation results of GLIM criteri have a good correlation with anthropometrical indexes and human body compositions.More comprehensive and accurate understanding of the nutritional status of the elderly could be acquired.GLIM has the potential to serve as a malnutrition screening tool for the elderly in communities.
