摘要
目的探讨生化检验对糖尿病临床诊断的价值。方法选取2020年3月—2021年3月于该院就诊的95例疑似糖尿病患者为研究对象,分别进行常规尿检、生化检验,以最终临床诊断结果为参考,对比分析常规尿检、生化检验在糖尿病临床诊断中的阳性预测值、阴性预测值、灵敏度、特异度;另比较分析糖尿病患者与非糖尿病患者的空腹血糖、餐后2 h血糖、糖化血红蛋白、三酰甘油。结果 95例患者中有84例为糖尿病,11例为非糖尿病;常规尿检结果显示,阳性预测值为88.16%,阴性预测值为10.53%,灵敏度为79.76%,特异度为18.18%;生化检验结果显示,阳性预测值为96.38%,阴性预测值为66.67%,灵敏度为95.24%,特异度为72.73%;两种检验方法比较,差异有统计学意义(P<0.05);另以糖尿病患者、非糖尿病患者生化指标对比,糖尿病患者的空腹血糖、餐后2 h血糖、糖化血红蛋白值、三酰甘油均高于非糖尿病患者,差异有统计学意义(P<0.05)。结论针对疑似糖尿病患者实施生化检验,可降低漏诊、误诊风险,同时明确患者的生化指标,有利于为患者的后期治疗提供有价值的参考。
Objective To explore the value of biochemical tests in the clinical diagnosis of diabetes. Methods A total of 95 suspected diabetic patients who were treated in the hospital from March 2020 to March 2021 were selected as study object,and routine urinalysis and biochemical tests were performed respectively, and the final clinical diagnosis results were used as reference. Comparative analysis of the positive predictive value, negative predictive value, sensitivity, and specificity of routine urine test and biochemical test in the clinical diagnosis of diabetes. In addition, the fasting blood glucose, 2 h postprandial blood glucose, glycosylated hemoglobin, and triacylglycerol of diabetic patients and non-diabetic patients were compared and analyzed. Results Of the 95 patients, 84 were diabetic and 11 were non-diabetic. Routine urine test results showed that the positive predictive value was 88.16%, the negative predictive value was 10.53%, the sensitivity was79.76%, and the specificity was 18.18%. The biochemical test results showed that the positive predictive value was 96.38%,the negative predictive value was 66.67%, the sensitivity was 95.24%, and the specificity was 72.73%. Comparison of two detection methods, the difference was statistically significant(P<0.05). In addition, compared the biochemical indicators of diabetic patients and non-diabetic patients, the fasting blood glucose, 2 h postprandial blood glucose, glycosylated hemoglobin value, and triglycerides of diabetic patients were higher than those of non-diabetic patients, and the difference was statistically significant(P <0.05). Conclusion The implementation of biochemical tests for patients with suspected diabetes can reduce the risk of missed diagnosis and misdiagnosis, and at the same time clarify the biochemical indicators of the patient, which is beneficial to provide valuable references for the patient’s later treatment.
