摘要
目的:探讨腹主动脉球囊阻断术在凶险性前置胎盘剖宫产术中的应用效果。方法:研究对象为2017年2月至2020年2月在郑州大学附属洛阳中心医院分娩的56例凶险性前置胎盘产妇,使用随机数字表法将其分为对照组(28例)和观察组(28例),对照组采取常规剖宫产术,观察组产妇在剖宫产术前行腹主动脉球囊阻断术。比较两组产妇术中指标(出血量、输血量及子宫切除率)和新生儿情况(体质量及窒息率)。结果:观察组产妇术中出血量及输血量均明显少于对照组,差异具有统计学意义(P<0.05)。观察组产妇的子宫切除率低于对照组,差异具有统计学意义(P<0.05)。两组新生儿体质量及窒息率无明显差异(P>0.05)。结论:对凶险性前置胎盘剖宫产术产妇采取腹主动脉球囊阻断术有助于减少术中出血量及输血量,降低子宫切除率,具有一定的安全性。
Objective To explore the effect of abdominal aortic balloon occlusion in cesarean section for dangerous placenta previa.Methods The subjects of the study were 56 women with dangerous placenta previa who gave birth at the Luoyang Central Hospital Affiliated to Zhengzhou University from February 2017 to February 2020.They were divided into a control group(28 cases)and an observation group(28 cases)using random number expression,the control group underwent conventional cesarean section,and the observing group received abdominal aortic balloon occlusion before cesarean section.Intraoperative indexes(bleeding volume,blood transfusion volume,and hysterectomy rate)and neonatal conditions(body weight and asphyxia rate)were compared between the two groups.Results The amounts of bleeding and blood transfusion in the observation group were significantly less than those in the control group,and the differences were statistically significant(P<0.05).The rate of hysterectomy in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the weight and asphyxia rate of newborns between the two groups(P>0.05).Conclusion For women undergoing cesarean section with dangerous placenta previa,abdominal aortic balloon occlusion can help reduce intraoperative blood loss and blood transfusion,reduce the rate of hysterectomy,and has a certain degree of safety.
