摘要
目的探讨妊娠期糖尿病(GDM)患者联用诺和锐与二甲双胍治疗的疗效及对妊娠结局的影响。方法选取该院2019年5月—2020年4月收治的84例GDM患者,以计算机软件SAS将其分成两组进行对比研究,各42例。对照组患者以诺和锐治疗,观察组在对照组基础上加用二甲双胍,对比两组患者治疗疗效、妊娠结局等。结果治疗后,对照组治疗总有效率为78.57%(33/42),观察组治疗总有效率97.62%(41/42),观察组高于对照组,差异有统计学意义(χ^(2)=7.265,P<0.05)。治疗前两组患者的血糖指标对比差异无统计学意义(P>0.05);治疗后观察组的空腹血糖、餐后2 h血糖降至(5.10±0.82)、(7.25±1.05)mmol/L,低于对照组的(5.74±0.85)、(8.10±1.21)mmol/L,差异有统计学意义(t=3.512、3.439,P<0.05)。观察组早产、新生儿呼吸窘迫、羊水增多等不良妊娠结局总发生率为4.76%(2/42),低于对照组的23.81%(10/42),差异有统计学意义(χ^(2)=6.222,P<0.05)。结论以诺和锐联合二甲双胍治疗GDM,可取得理想疗效,对患者的血糖指标改善明显;并且可显著降低不良妊娠结局发生的风险。
Objective To explore the effect of insulin aspart injection combined with metformin in the treatment of gestational diabetes mellitus(GDM) and its influence on pregnancy outcome.Methods A total of 84 patients with GDM admitted to the hospital from May 2019 to April 2020 were selected and divided into two groups by computer software SAS for comparative study,each with 42 cases.The control group was treated with insulin aspart,and the observation group was treated with metformin on the basis of the control group,compared the treatment efficacy and pregnancy outcome of the two groups.Results After treatment,the total effective rate of the control group was 78.57%(33/42),and the total effective rate of the observation group was 97.62%(41/42),which was higher than that of the control group,the difference was statistically significant(χ^(2)=7.265,P<0.05).Before treatment,there was no significant difference in blood glucose index between the two groups of patients( P > 0.05);after treatment,the fasting blood glucose of the observation group and the 2 h postprandial blood glucose decreased to(5.10 ±0.82)mmol/L,(7.25 ±1.05)mmol/L,which was lower than the control group(5.74 ±0.85)mmol/L,(8.10 ±1.21)mmol/L,the difference was statistically significant(t=3.512,3.439,P<0.05).The total incidence of adverse pregnancy outcomes such as preterm birth,neonatal respiratory distress,and increased amniotic fluid in the observation group was4.76%(2/42),which was lower than 23.81%(10/42) of the control group,the difference was statistically significant(χ^(2)=6.222,P<0.05).Conclusion Insulin aspart injection combined with metformin in the treatment of GDM can achieve an ideal effect and significantly improve the patient’s blood glucose index;and it can significantly reduce the risk of adverse pregnancy outcomes.
