摘要
目的探究腹腔镜下完整全结肠系膜切除治疗右半结肠癌的临床疗效。方法选取2019年5月—2020年5月该院收治的88例右半结肠癌患者为研究对象,以随机数字表法将其分为研究1组(腹腔镜下完整全结肠系膜切除术)与研究2组(腹腔镜右半结肠癌根治术)各44例,探讨实施不同治疗对治疗效果的影响。结果与研究2组相比,研究1组术中失血量较少,手术时间、住院时间较短,淋巴结清扫数较多,差异有统计学意义(P<0.05)。研究1组并发症发生率4.55%低于研究2组20.45%,差异有统计学意义(χ^(2)=5.091,P<0.05)。术前研究对象QOL评分相比,差异无统计学意义(P>0.05);术后研究1组各项评分均高于研究2组,差异有统计学意义(P<0.05)。研究对象术前PSQI评分比较,差异无统计学意义(P>0.05);术后研究1组评分明显高于研究2组,差异有统计学意义(P<0.05)。结论完整全结肠系膜切除在缩短患者手术时间、降低术中出血量及并发症发生率方面发挥积极作用,还可显著改善患者生活质量与睡眠治疗,促进预后康复,此方法值得应用与推广。
Objective To explore the clinical effect of laparoscopic complete mesocolonectomy in the treatment of right colon cancer.Methods 88 patients with right colon cancer treated in the hospital from May 2019 to May 2020 were selected as the research object.They were randomly divided into study group 1(laparoscopic complete mesocolon excision)and study group 2(laparoscopic radical resection of right colon cancer),with 44 cases in each group,to explore the impact of different treatments on the treatment effect.Results Compared with study group 2,study group 1 had less intraoperative blood loss,shorter operation time and hospital stay,and more lymph node dissection,the difference was statistically significant(P<0.05).The incidence of complications in study 1 group was 4.55%,lower than 20.45%in study 2 group,the difference was statistically significant(χ^(2)=5.091,P<0.05).There was no statistically significant difference in QOL score before operation(P>0.05);the scores of study group 1 were higher than those of study group 2,the difference was statistically significant(P<0.05).There was no statistically significant difference in preoperative PSQI score(P>0.05);the score of study group 1 was significantly higher than that of study group 2,the difference was statistically significant(P<0.05).Conclusion Complete total mesocolon resection plays a positive role in shortening the operation time of patients,reducing intraoperative blood loss and complication rate.It can also significantly improve the quality of life and sleep therapy of patients,and promote prognostic recovery.This method is worthy of application and promotion.
