摘要
目的分析人乳头瘤病毒(HPV)、液基薄层细胞学(TCT)及DNA倍体联合筛查在宫颈癌患者诊断中的临床应用价值。方法选取本院256例宫颈病变患者,均行HPV、TCT、DNA倍体检查,以病理活检为"金标准",比较三种检查方式单独及联合对宫颈癌的诊断敏感度、特异度、准确性。结果各方式单独诊断中,DNA倍体分析诊断效能最高;联合诊断中,TCT+DNA倍体Kappa值最高,为0.79;HPV+TCT、TCT+DNA诊断敏感度明显高于HPV+DNA(P<0.05);HPV+DNA诊断特异度高于HPV+TCT、TCT+DNA(P<0.05),TCT+DNA特异度高于HPV+TCT(P<0.05);TCT+DNA、HPV+DNA诊断准确性高于HPV+TCT(P<0.05)。结论相较于HPV、TCT,DNA倍体对宫颈癌诊断价值更高,其联合HPV与TCT能够有效提高诊断准确性。
Objective To analyze the clinical application value of joint screening of human papilloma virus(HPV),thinprep cytology test(TCT)and DNA ploidy in the diagnosis of cervical cancer.Methods We selected 256 patients with cervical lesions and performed examinations of HPV,TCT,and DNA ploidy on these patients.Biopsy was taken as the golden standard to compare sensitivity,specificity,and accuracy of the three examination methods alone and in combination for diagnosing cervical cancer.Results For single method,the diagnostic efficiency of DNA ploidy analysis was the highest.For combined method,the Kappa value of TCT+DNA ploidy was the highest(0.79).The diagnostic sensitivity of HPV+TCT and TCT+DNA ploidy analysis was significantly higher than that of HPV+DNA ploidy analysis(P<0.05).The diagnostic specificity of HPV+DNA ploidy analysis was higher than that of HPV+TCT and TCT+DNA ploidy analysis(P<0.05).The specificity of TCT+DNA ploidy analysis was higher than that of HPV+TCT(P<0.05).The diagnostic accuracy of TCT+DNA ploidy analysis and HPV+DNA ploidy analysis was higher than that of HPV+TCT(P<0.05).Conclusion Compared with HPV and TCT,DNA ploidy is more valuable in the diagnosis of cervical cancer.The combined use with HPV and TCT can effectively improve the diagnostic accuracy.
