摘要 目的:探讨3.0T磁共振T2^(*)mapping成像分区定量评估膝关节软骨的应用价值。方法:回顾性分析本院30例膝关节软骨损伤的患者的常规MRI和T2^(*)mapping影像资料,将股骨、胫骨平台及髌骨软骨分为14个亚区,共获取420个软骨亚区,通过矢状位质子密度加权成像(PDWI)脂肪抑制序列图像分析软骨形态和信号改变,将420个软骨亚区分为正常组、轻度损伤组及重度损伤组,另将正常组分为承重区及非承重区,测量其T2^(*)值并进行对照分析。结果:软骨正常组332个,轻度损伤组41个,重度损伤组47个。轻度及重度损伤组的T2^(*)平均值均高于正常组(P<0.05),重度损伤组的T2^(*)平均值高于轻度损伤组,但2组间差异无统计学意义(P=0.12)。另外,正常组的承重区和非承重区的T2^(*)值差异具有统计学意义(P<0.05)。结论:T2^(*)mapping成像可用于定量评估膝关节软骨,能更直观地区分正常和损伤的软骨,对膝关节软骨损伤的定量诊断具有重要的临床应用价值。 Purpose:To investigate the application value of knee cartilage quantitative evaluation using 3.0 T MR T2^(*)mapping imaging.Methods:The routine MRI and T2^(*)mapping imaging data of 30 patients with knee cartilage injury in our hospital were analyzed retrospectively.According to Hunter’s method,the femur,tibial plateau and patellar cartilage were divided into 14 sub areas,and 420 sub areas of cartilage were obtained.By analyzing cartilage morphology and signal changes in proton density weighted imaging(PDWI),420 cartilage subsets were divided into normal group,mild injury group and severe injury group,and the normal group was divided into load-bearing area and non-load-bearing area.T2^(*)values were measured and compared.Results:There were 332 sub areas in normal group,41 in mild injury group and 47 in severe injury group.The mean values of T2^(*)in the mild and severe injury group were both higher than that in the normal group(P<0.05).The mean value of T2^(*)in the severe injury group was higher than that in the mild injury group,but there was no statistical difference between the two groups(P=0.12).In addition,the T2^(*)values of load-bearing area and non-load bearing area in the normal group were with statistically significant difference(P<0.05).Conclusion:T2^(*)mapping can be used to quantitatively evaluate the cartilage of knee joint and distinguish the normal and damaged cartilage more intuitively.It has an important clinical application value for the diagnosis of knee cartilage injury.
