围术期血糖管理对糖尿病患者骨折手术的效果评价 被引量:1

摘要 目的探究加强围术期血糖管理对骨折手术合并糖尿病患者的效果。方法该次研究对象从2018年4月—2020年6月在该院住院治疗的骨折手术合并糖尿病患者中选取80例。按照随机数表法将所有患者分为两组,对照组40例应用常规围术期血糖管理,观察组40例则加强围术期血糖管理,对比两组患者的血糖指标,骨折愈合情况和手术伤口愈合情况及时间,GIP(葡萄糖依赖性促胰岛素多肽)、GLP-1(胰高血糖素样肽-1)、C-肽指标,术中指标,术后并发症发生率。结果观察组空腹血糖、餐后2 h血糖、糖化血红蛋白值明显低于对照组,差异有统计学意义(P<0.05);观察组GIP、GLP-1、C-肽指标明显高于对照组,差异有统计学意义(P<0.05);观察组骨折愈合率明显高于对照组,差异有统计学意义(P<0.05);观察组手术伤口愈合率明显高于对照组,差异有统计学意义(P<0.05);观察组术中出血量明显少于对照组,手术时间明显短于对照组,差异有统计学意义(P<0.05);观察组并发症发生率明显少于对照组,差异有统计学意义(P<0.05)。结论对骨折手术合并糖尿病患者加强围术期血糖管理具有显著的效果,能将血糖控制在更加合理的范围内,不但能缩短骨折、手术伤口的愈合时间,还能降低术中出血量、术后并发症率,有利于患者的恢复。 Objective To explore the effect of strengthening perioperative blood glucose management on patients with fracture surgery combined with diabetes mellitus.Methods The subjects of this study were selected from 80 patients with fracture surgery complicated with diabetes who were hospitalized in the hospital from April 2018 to June 2020.All patients were divided into 2 groups according to random number table method.40 patients in the control group received routine perioperative blood glucose management,and 40 patients in the observation group received enhanced perioperative blood glucose management.Blood glucose index,the fracture healing status,surgical wound healing status and time,GIP(glucose-dependent insulin stimulating polypeptide),GLP-1(glucagon-like peptide-1),C-peptide indicators,intraoperative indicators,and incidence of postoperative complications were compared between the two groups.Results The fasting blood glucose,two hours postprandial blood glucose and glycosylated hemoglobin of the observation group were significantly lower than those of the control group,the difference was statistically significant(P<0.05);GIP,GLP-1 and C-peptide in the observation group were significantly higher than those in the control group,the difference was statistically significant(P<0.05);the fracture healing rate of the observation group was significantly higher than that of the control group,the difference was statistically significant(P<0.05);the wound healing rate of the observation group was significantly higher than that of the control group,the difference was statistically significant(P<0.05);the intraoperative blood loss of the observation group was significantly less than that of the control group,and the operation time of the observation group were significantly shorter than those of the control group,the difference was statistically significant(P<0.05);the incidence of complication of the observation group was significantly less than that of the control group,the difference was statistically significant(P<0.05).Conclusion It is significant to strengthen perioperative blood glucose management in patients with fracture surgery combined with diabetes.It can control blood glucose in a more reasonable range,not only shorten the healing time of fracture and surgical wound,but also reduce intraoperative blood loss and postoperative complication rate,which is beneficial to the recovery of patients.
