摘要
目的探讨以临床路径为基础的精细护理(CNP-PN)在肺癌患者围手术期的应用效果。方法选取2018年12月至2019年6月于唐河县人民医院行肺癌根治术的60例肺癌患者为对照组。选取2019年7—12月于唐河县人民医院行肺癌根治术的60例肺癌患者为观察组。给予对照组患者围手术期常规护理,给予观察组患者CNP-PN干预。比较两组患者手术指标、并发症及护理满意度。比较两组干预前后汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)、癌因性疲乏、生活质量的评分。结果观察组下床时间、排气时间、住院时间短于对照组(P<0.05)。干预后,两组HAMA、HAMD、癌因性疲乏评分低于干预前,观察组HAMA、HAMD、癌因性疲乏评分低于对照组(P<0.05)。干预后,两组生活质量评分高于干预前,观察组生活质量评分高于对照组(P<0.05)。观察组并发症发生率较对照组低,观察组护理满意度较对照组高(P<0.05)。结论CNP-PN可有效缩短肺癌患者根治术后康复时间,提高生活质量及护理满意度,改善心理状态,减少并发症。
Objective To explore the application effect of fine nursing based on clinical pathway(CNP-PN)in perioperative period of patients with lung cancer.Methods Sixty patients with lung cancer who underwent radical resection of lung cancer at Tanghe County People’s Hospital from December 2018 to June 2019 were selected as control group.Sixty lung cancer patients who underwent radical resection of lung cancer at Tanghe County People’s Hospital from July to December 2019 were selected as observation group.The patients in control group were given routine care during the perioperative period,and CNP-PN intervention was given to the patients in observation group.The surgical indicators,complications and nursing satisfaction of the two groups of patients were compared.The scores of Hamilton anxiety scale(HAMA),Hamilton depression scale(HAMD),cancer-related fatigue and quality of life were compared between the two groups before and after intervention.Results The time to get out of bed,exhaust time and hospital stay in observation group were shorter than those in control group(P<0.05).After intervention,the scores of HAMA,HAMD and cancer-related fatigue in the two groups were lower than those before intervention,and the scores of HAMA,HAMD and cancer-related fatigue in observation group were lower than those in control group(P<0.05).After intervention,the score of quality of life in the two groups was higher than that before intervention,and the score of quality of life in observation group was higher than that in control group(P<0.05).The incidence of complications in observation group was lower than that in control group,and the nursing satisfaction in observation group was higher than that in control group(P<0.05).Conclusion CNP-PN can effectively shorten the recovery time of patients with lung cancer after radical surgery,improve the quality of life and nursing satisfaction,improve the psychological state and reduce complications.
