摘要
目的观察通络壮骨丸治疗肝肾亏虚型膝关节骨性关节炎(KOA)的临床疗效。方法选取2018年1月至2020年12月在湖北中医药大学附属公安县中医医院住院的肝肾亏虚型KOA患者作为研究对象,按照要求共纳入68例,采用随机数字表法分为观察组及对照组,各34例。对照组患者均采用常规治疗联合使用双醋瑞因胶囊进行治疗,观察组所有患者在对照组的治疗基础上加用自制通络壮骨丸,均服用8周。在治疗第4周和第8周对两组患者的美国西部Ontario和McMaster大学骨关节炎指数(WOMAC)评分、视觉模拟(VAS)评分、中医证候评分进行比较分析;对治疗8周后的治疗有效率进行分析。结果治疗后4周两组患者的WOMAC评分、VAS评分、中医证候评分较治疗前明显降低(P<0.05);治疗后8周两组患者的WOMAC评分、VAS评分、中医证候评分较治疗后4周明显降低(P<0.05);在治疗后4、8周两个时间点,观察组各评分低于对照组,差异均有统计学意义(P<0.05);观察组的治疗总有效率为91.18%,明显高于对照组的70.58%(P<0.05)。结论自制通络壮骨丸和双醋瑞因胶囊均能有效的治疗KOA,但与单纯应用双醋瑞因胶囊相比,联合应用自制通络壮骨丸能较快缓解相关症状、改善中医证候评分。
Objective To observe the clinical efficacy of Tongluo Zhuanggu Pills in treating knee osteoarthritis of liver and kidney deficiency type.Methods Patients with knee osteoarthritis of liver and kidney deficiency who were hospitalized in Gong'an County Hospital of traditional Chinese Medicine Affiliated to Hubei University of traditional Chinese Medicine from January 2018 to December 2020 were selected as the research objects.A total of 68 cases were included in accordance with the requirements,and were divided into 34 cases in the observation group and 34 cases in the control group by random number table method.The patients in the control group were treated with conventional treatment combined with diacerein capsules.All patients in the observation group were treated with self-made Tongluo Zhuanggu Pills on the basis of the treatment of the control group,and they were all treated for 8 weeks.In the 4th and 8th weeks of treatment,the Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)scores,visual analog scores(VAS),and TCM syndrome scores were compared and analyzed between the two groups of patients;The effective rate after 8 weeks of treatment was analyzed.Results The WOMAC scores,VAS scores,and TCM syndrome scores of the two groups of patients at 4 weeks after treatment were significantly lower than those before treatment(P<0.05);The WOMAC scores,VAS scores,and TCM syndrome scores of the two groups of patients at 8 weeks after treatment were significantly lower than those at 4 weeks after treatment(P<0.05);The scores of the two groups at 4 weeks after treatment were compared with those of the two groups at 8 weeks after treatment,and the scores of the observation group were lower than those of the control group,with the differences statistically significant(P<0.05).The total effective rate of treatment in the observation group was 91.18%,which was significantly higher than that in the control group(70.58%),(P<0.05).Conclusion Both self-made Tongluo Zhuanggu Pills and diacerein capsules can effectively treat knee osteoarthritis.However,compared with the single administration of diacerein capsules,the combined application of self-made Tongluo Zhuanggu Pills and diacerein capsules can relieve related symptoms and improve the TCM syndr ome scores faster.
