摘要
目的探讨微创撬拨复位空心钉与锁定钢板内固定对跟骨骨折术后康复的影响。方法选择2018年2月至2020年2月中山大学附属第三医院粤东医院收治的跟骨骨折患者84例,按随机数字表法将其分为两组,每组各42例。对照组采用锁定钢板内固定术治疗,观察组采用微创撬拨复位空心钉内固定术治疗。比较两组术中指标、疼痛程度、足部功能及并发症发生率。结果观察组患者的出血量(13.29±4.81)ml少于对照组的(50.64±10.38)ml,手术时间(53.64±12.17)min短于对照组的(98.75±16.49)min,Maryland足部功能评分(80.03±7.12)分高于对照组(74.84±8.29)分,术后1、7 d时视觉模拟评分法(VAS)评分(4.59±1.07)分、(2.73±0.59)分及并发症总发生率(2.38%)低于对照组的(5.74±1.12)分、(4.05±0.64)分、19.05%,差异有统计学意义(P<0.05)。结论跟骨骨折采用微创撬拨复位空心钉内固定术治疗安全可靠,手术损伤小,在减少并发症发生率、缓解患者疼痛及加快术后足部功能恢复方面效果理想。
Objective To investigate the effect of minimally invasive prying reduction and hollow nail internal fixation and locking plate internal fixation on postoperative rehabilitation of calcaneal fractures.Methods A total of 84 patients with calcaneal fractures treated in Yuedong Hospital of the Third Affiliated Hospital of Sun Yat-Sen University from February 2018 to February 2020 were selected.They were divided into two groups according to the random number table method,with 42 patients in each group.The control group was treated with locking plate internal fixation,while the observation group was treated with minimally invasive prying reduction and hollow nail internal fixation.The intraoperative indices,pain level,foot function and complications rate were compared between the two groups.Results The blood loss of patients in the observation group(13.29±4.81)ml was less than that of the control group(50.64±10.38)ml,and the operation time in the observation group(53.64±12.17)min was shorter than that of the control group(98.75±16.49)min.The Maryland score in the observation group(80.03±7.12)was higher than that of the control group(74.84±8.29).The scores of Visual Analogue Scale/Score(VAS)at 1 d and 7 d after operation were[(4.59±1.07)and(2.73±0.59)]in the observation group,which were lower than those in the control group[(5.74±1.12)and(4.05±0.64)],respectively.The total complication rate in the observation group(2.38%)was lower than that of the control group(19.05%).There were statistically significant differences between the two groups in the above-mentioned indices(P<0.05).Conclusion In the treatment of calcaneal fractures,minimally invasive prying reduction and hollow nail internal fixation is safe and reliable,with less operative injury.It has ideal efficacy in reducing complications rate,relieving pain and accelerating po stoperative foot function recovery.
