摘要
由于胰腺癌容易出现门静脉系统侵犯,导致总体手术R0切除率低,国内外指南现已将联合静脉切除重建的胰腺癌根治术作为治疗门静脉系统受侵的胰腺癌的重要方法。然而,对于门静脉系统受侵的分型方法、血管的重建方式和血管替代材料的选择,目前均无相应的临床应用标准。为此,中国医疗保健国际交流促进会肝脏移植学分会、中国医疗保健国际交流促进会肾脏移植学分会、中华预防医学会肝胆胰疾病预防与控制专业委员会和中国研究型医院学会普通外科专业委员会组织国内相关专家制订本推荐意见,旨在为异体静脉置换技术在胰腺癌合并门静脉系统受侵中应用提供指导性建议。
Due to the early invasion to the portal vein system,the overall R0 resection rate of pancreatic cancer is pretty low.Therefore,radical resection of the pancreatic cancer combined with venous resection and reconstruction has been recommended as an important procedure for the treatment of portal vein invasion in patients with pancreatic cancer based on the guidelines at home and abroad.However,there has been no consensus on the classifications of portal vein invasion or the types and the vascular substitute materials of vascular reconstruction.To this end,the Liver Transplantation Branch of China International Exchange and Promotive Association for Medical and Health Care,Kidney Transplantation Branch of China International Exchange and Promotive Association for Medical and Health Care,Hepatobiliary Pancreatic Disease Prevention and Control Committee of Chinese Preventive Medicine Association and General Surgery Committee of Chinese Research Hospital Association organized relevant domestic experts to provide the recommendations,aiming at providing the guidance for the application of allograft vein reconstruction in pancreatic cancer patients with portal vein invasion.
