超声引导下经皮球囊扩张血管成形术治疗自体动静脉内瘘狭窄的自身对照研究 被引量:27

A self-controlled study of ultrasound-guided percutaneous transluminal angioplasty in the treatment of autogenous arteriovenous fistula stenosis

摘要 目的:评估超声引导下经皮球囊扩张血管成形术(Percutaneous transluminal angioplasty,PTA)治疗自体动静脉内瘘(Autogenous arteriovenous fistula,AVF)狭窄的疗效。方法:收集2020年8月—2021年3月于我院行超声引导下PTA治疗AVF狭窄的患者24例,将其术前与术后超声测量的狭窄处内径、峰值血流速度、肱动脉血流量及透析时泵控血流量进行自身对照研究。结果:24例AVF狭窄的患者均行球囊扩张成功,PTA术后狭窄处内径、肱动脉血流量、透析时泵控血流量均较术前显著增加(P<0.05);PTA术后峰值血流速度较术前明显降低(P<0.05)。行PTA术后患者均能应用该瘘顺利完成透析。结论:应用超声引导下PTA治疗AVF狭窄的疗效显著,可以很好的满足临床需要,且安全易行,具有重要临床价值。 Objective:To evaluate the efficacy of ultrasound-guided percutaneous transluminal angioplasty(PTA)in the treatment of autogenous arteriovenous fistula(AVF)stenosis.Methods:A total of 24 patients with AVF stenosis treated by ultrasound-guided PTA in our hospital from August 2020 to March 2021 were collected.The stenosis internal diameter,peak blood flow velocity,brachial artery blood flow measured by ultrasound and blood pump flow before and after operation were self controlled study.Results:Balloon dilatation was performed successfully in 24 patients with AVF stenosis.The internal diameter of stenosis,brachial artery blood flow and blood pump flow during dialysis after PTA were significantly higher than those before operation(P<0.05).The peak blood flow velocity after PTA was significantly lower than that before operation(P<0.05).The fistula can be successfully used to complete dialysis after PTA.Conclusion:Ultrasound-guided PTA is effective in the treatment of AVF stenosis,which can well meet the clinical needs,and is safe and easy,and has important clinical value.
