摘要
目的探讨数字乳腺断层融合X线摄影(DBT)对乳腺单纯结构扭曲良恶性病变的诊断价值。方法回顾性分析2016年5月-2019年12月扬州市妇幼保健院经病理证实的91例乳腺单纯结构扭曲患者的临床资料,比较DBT与FFDM对乳腺单纯结构扭曲的检出率及诊断效能,分析DBT及全数字化乳腺X线摄影(FFDM)对乳腺单纯结构扭曲良恶性病变的诊断价值。结果病理证实恶性病变37例,良性病变54例;91例患者中致密性乳腺74例,非致密性乳腺17例,良性病变54例;恶性病变37例,DBT对乳腺单纯结构扭曲的检出率为91.21%,高于FFDM的41.76%,差异有统计学意义(P<0.05);DBT对乳腺单纯结构扭曲的诊断敏感性、特异性及诊断符合率均优于FFDM,差异有统计学意义(P<0.05)。结论DBT能显著提高乳腺内特别是致密性乳腺内单纯结构扭曲的检出率及诊断敏感性、特异性和诊断符合率,为临床诊治提供更可靠的依据。
Objective To investigate the diagnostic value of digital breast tomographic fusion radiography(DBT)in benign and malignant lesions of simple structural distortion of breast.Methods The clinical data of 91 patients with simple structural distortion of breast confirmed by pathology in Yangzhou maternal and child health hospital from May 2016 to December 2019 were analyzed retrospectively.The detection rate and diagnostic efficacy of DBT and FFDM were compared,and the diagnostic value of DBT and full digital mammography(FFDM)in benign and malignant lesions of simple structural distortion of breast was analyzed.Results 37 cases of malignant lesions and 54 cases of benign lesions were confirmed by pathology;Among the 91 patients,there were 74 cases of dense breast,17 cases of non dense breast and 54 cases of benign lesions;In 37 cases of malignant lesions,the detection rate of simple structural distortion of breast by DBT was 91.21%,which was higher than 41.76%of FFDM(P<0.05);The sensitivity,specificity and coincidence rate of DBT in the diagnosis of simple structural distortion of breast were better than FFDM(P<0.05).Conclusion DBT can significantly improve the detection rate,diagnostic sensitivity,specificity and diagnostic coincidence rate of simple structural distortion in breast,especially in dense breast,and provide a more reliable basis for clinical diagnosis and treatment.
