摘要
目的:探究氢氧化钙糊剂活髓保存治疗龋源性牙髓炎对患儿LTB4、TNF-α、IL-8水平及咬合功能的影响。方法:选择2018年1月-2019年3月笔者医院收治的130例龋源性牙髓炎患儿为研究对象,通过计算机产生随机数法分为观察组(n=65,患牙数69颗)和对照组(n=65,患牙数67颗)。两组均接受活髓保存治疗,对照组使用氧化锌丁香油水门汀,观察组使用氢氧化钙糊剂。比较两组治疗1年后的疗效以及治疗后7d两组疼痛、龈沟液炎症因子及咬合功能。结果:治疗后1年,观察组治愈率高于对照组(94.20% vs 73.13%,P<0.05);治疗后7d,两组VAS、FPS-R评分均下降,且观察组低于对照组(P<0.05);治疗后7d,两组龈沟液LTB4、TNF-α及IL-8水平降低,且观察组均低于对照组(P<0.05);治疗后7d,两组进食不便评分、口腔软组织不适评分、语言障碍程度评分均下降,且观察组低于对照组(P<0.05)。结论:与氧化锌丁香油水门汀比较,氢氧化钙糊剂活髓保存可更有效地降低龋源性牙髓炎患儿血清LTB4、TNF-α及IL-8水平,控制炎症反应,同时抑制疼痛,改善咬合功能,疗效确切,值得临床推广应用。
Objective To explore the effect of calcium hydroxide paste on the expression of LTB4,TNF-α,IL-8 and occlusal function in children with carious pulpitis.Methods 130 children with carious pulpitis from January 2018 to March 2019 were selected,and randomly divided into the observation group(n=65,number of teeth affected 69)and the control group(n=65,number of teeth affected 67).Both groups were treated with vital pulp preservation,the control group with zinc oxide clove oil paste,the observation group with calcium hydroxide paste.The pain,gingival crevicular fluid inflammatory factors and occlusal function of the two groups were compared before and 7 days after treatment.Result One year after treatment,the cure rate of the observation group was significantly higher than that of the control group(94.20%vs 73.13%,P<0.05).On the 7th day after treatment,the VAS and FPS-R scores of the two groups were significantly lower than those of the control group(P<0.05).On the 7th day after treatment,the gingival crevicular fluid levels of LTB4,TNF-αand IL-8 in the two groups decreased,and the observation group was significantly lower than the control group(P<0.05).On the 7th day after treatment,the scores of eating inconvenience,oral soft tissue discomfort and language disorder in the two groups were significantly lower than those in the control group(P<0.05).Conclusion Compared with zinc oxide clove oil paste,calcium hydroxide paste can reduce the expression level of serum LTB4,TNF-αand IL-8 in carious pulpitis children,control the inflammatory response,inhibit pain,improve the occlusal function,with definite effect,which is worthy of clinical promotion.
