摘要
目的探讨开放式乳突切开术联合鼓室成形术在慢性化脓性中耳炎及中耳胆脂瘤治疗中的应用及疗效。方法选择江苏省昆山市第一人民医院耳鼻喉科于2017年1月~2019年1月期间行开放式乳突切开术联合鼓室成形术的慢性化脓性中耳炎及中耳胆脂瘤患者71例,统计术后干耳时间、听力情况,按照听力重建方式不同进行回顾性分析。结果 71例患者术后平均干耳时间为(27.96±5.97) d。其中行自体材料听力重建30例、人工钛听小骨听力重建41例。自体材料组术前平均听阈为(55.31±18.01) dB HL,术后6个月为(44.00±19.05) dB HL;人工钛听小骨组术前平均听阈为(55.44±21.97) dB HL,术后6个月为(40.42±20.23) dB HL,自体材料组及人工钛听小骨组重建听力前后的听阈改变差异均有统计学意义。结论开放式乳突切开术联合鼓室成形术治疗慢性化脓性中耳炎及中耳胆脂瘤效果明显,乳突缩窄利于干耳,上鼓室加高可以提高上鼓室含气量,提高听力;另外,自体软骨取材方便,适用于经济困难的患者。
Objective To investigate the effect of open mastoidectomy and tympanoplasty on the treatment of chronic suppurative otitis media and cholesteatoma. Methods Seventy-one patients with chronic suppurative otitis media and middle ear cholesteatoma who had undergone open mastoidectomy combined with tympanoplasty from January 2017 to January 2019 in the Department of Otorhinolaryngology of Kunshan First People’s Hospital of Jiangsu Province were selected. The postoperative dry ear time and hearing condition were analyzed. A retrospective analysis was made according to the different ways of hearing reconstruction. Results The average postoperative dry ear time of 71 patients were(27.96 ± 5.97) days. Among them, 30 cases were reconstructed with autogenous materials and 41 cases with artificial titanium ossicles. The average hearing threshold was(55.31 ± 18.01) dB HL before operation and(44.00 ± 19.05) dB HL at 6 months after operation in the autogenous material group, and(55.44 ± 21.97) dB HL and(40.42 ± 20.23) dB HL in the titanium ossicle group before and after the operation. Conclusions Open mastoidectomy combined with tympanoplasty is effective in the treatment of chronic suppurative otitis media and middle ear cholesteatoma. Mastoid constriction is beneficial to dry ears, and the elevation of upper tympanic cavity can improve the air content of upper tympanic cavity and improve hearing. Autologous cartilage is easy to obtain and suitable for patients with financial difficulties.
