摘要
目的基于真实世界电子医疗数据库分析脑梗死患者使用刺五加注射液的临床应用特征。方法该研究采用数据挖掘方法中的关联规则对全国16家大型三级甲等医院信息系统(hospital information system,HIS)采集的电子医疗数据进行回顾性分析,针对诊断为脑梗死且用刺五加注射液治疗的住院患者,获得其一般情况、诊断信息、西药联合用药等相关信息,通过统计描述与Apriori算法分析联合用药关联性。结果使用刺五加注射液的脑梗死患者共2376例,以45岁以上的中老年人为主(2311例,97.26%),男(1302例,58.10%)多于女(939例,41.90%),常合并有高血压、冠心病、高脂血症、糖尿病等疾病,临床与单硝酸异山梨酯、美托洛尔、硝苯地平、硝酸异山梨酯、乙酰水杨酸等药物联合使用较为常见。药理方面,主要与β受体阻断药、钙通道阻断药、心脏病治疗药、血管保护药、祛瘀剂类等联合使用。结论真实世界中刺五加注射液脑梗死人群以中老年人为主,男性多于女性,临床用药方案表现为常与β受体阻断药、钙通道阻断药、心脏病治疗药及血管保护药等联合使用,治疗脑梗死疾病的疗效真实,但由于该研究采用回顾性的数据挖掘与分析方法,存在一定的局限性,建议临床医生谨慎参考,加强中医诊断与辨证施治,严格按照指南与药品说明书使用刺五加注射液。
Objective Based on the real-world electronic medical database,to analyze the clinical application characteristics of Ciwujia Injection(刺五加注射液) in the treatment of cerebral infarction patients.Methods This study used association rules in data mining method to retrospectively analyze the electronic medical data collected by information system(hospital information system,HIS) of 16 grade 3 A hospitals in China.For inpatients diagnosed as cerebral infarction and treated with Ciwujia Injection, the general situation,diagnosis information,western medicine combined medication and other related information were obtained,and thecorrelation and contrast visual graph were analyzed by statistical description and Apriori algorithm. Results A total of 2376 patients with cerebral infarction who used Ciwujia Injection were mainly aged over 45 years old( 2311 cases,97. 26%),men( 1302 cases,58. 10%) were more than women( 939 cases,41. 90%),often complicated with hypertension,coronary heart disease,hyperlipidemia,diabetes and other diseases. As for pharmacological aspects,it is mainly combined with β receptor blocker,calcium channel blocker,heart disease treatment,vascular protection and blood stasis removing agent. Conclusion In the real world,the cerebral infarct population of Ciwujia Injection is mainly middle-aged and elderly,male is more than female,and the clinical medication plan is often combined with β receptor blocker,calcium channel blocker,heart disease treatment and vascular protection drug. But because this study uses retrospective data mining and analysis method,there are some limitations. It is suggested that clinicians should carefully refer to it,strengthen the diagnosis and treatment of TCM syndrome differentiation,and use Ciwujia Injection strictly in accordance with the guidelines and drug instructions.
