摘要
目的探讨64排多层螺旋CT(MSCT)对急性胸腹部创伤的诊断价值。方法回顾性分析2018年3月至2020年1月开封市祥符区第一人民医院收治的86例急性胸腹部创伤患者,均行64排MSCT、数字化X射线摄影(DR)检查。比较64排MSCT、DR对急性胸腹部创伤的检出率及64排MSCT图像质量,评估64排MSCT对不同类型急性胸腹膜创伤的诊断价值。结果手术结果显示,86例急性胸腹部创伤患者中脾挫裂伤29例(33.72%),肝挫裂伤15例(17.44%),消化道破裂8例(9.30%),肾挫裂伤14例(16.28%),肋骨骨折20例(23.26%)。64排MSCT对不同类型急性胸腹膜创伤检出率高于DR(87.21%比68.60%,P<0.05);64排MSCT对不同类型急性胸腹膜创伤与手术诊断结果具有较高一致性(Kappa=0.836);64排MSCT图像质量优于DR(P<0.05)。结论64排MSCT应用于急性胸腹部创伤患者可提高检出率,且图像质量较高,为临床评估患者病情严重程度提供更多影像学依据。
Objective To investigate the diagnostic value of 64-multislice spiral CT(MSCT)in acute chest and abdominal trauma.Methods From March 2018 to January 2020,a total of 86 patients with acute chest and abdominal trauma in the First People’s Hospital of Kaifeng Xiangfu District were retrospectively analyzed.All patients were diagnosed by 64-MSCT and digital X-ray(DR).The detection rate of 64-MSCT and DR in the diagnosis of acute thoracic and abdominal trauma and the image quality of 64-MSCT were compared,and the uniformity between 64-MSCT and the surgical results in peritoneal trauma were evaluated.Results Among the 86 patients with acute chest and abdomen,there were 29 cases(33.72%)had spleen contusion,15 cases(17.44%)had liver contusion,8 cases(9.30%)had digestive tract rupture,14 cases(16.28%)had kidney contusion,and 20 cases(23.26%)had rib fracture.The detection rate of 64-MSCT in different types of acute thoracic and peritoneal trauma was higher than that of DR(87.21%vs.68.60%,P<0.05).The 64-MSCT diagnosis of acute thoracic and peritoneal trauma had a high consistency with the surgical diagnosis(Kappa=0.836).The image quality of 64-MSCT was better than DR(P<0.05).Conclusion The application of 64-MSCT in patients with acute thoracic and abdominal trauma can improve the detection rate,and the image quality is high,which can provide more imaging basis for clinical evaluation of the severity of the patient’s condition.
