摘要
目的探讨儿童时期滤泡淋巴瘤的临床表现、病理形态、治疗方法及预后等方面的特征。方法回顾性研究2009年2月—2020年7月我院诊治的滤泡淋巴瘤患儿的临床资料,包括起病年龄、临床表现、临床分期、组织学形态、治疗方案及预后。结果8例儿童均为男性,中位发病年龄为5岁4个月,起病部位在淋巴结3例,淋巴结外部位5例,分期Ⅰ期1例,Ⅱ期3例,Ⅲ期4例;组织学分级儿童型3例,普通型Ⅲ级5例,均予以化疗,其中5例联合利妥昔单抗治疗,治疗效果好。结论儿童时期滤泡淋巴瘤与成人不同,男性多见,无骨髓浸润,有其特有的病理形态及免疫表型,临床分期多以Ⅰ-Ⅲ期为主,病理分级以Ⅲ级为主,预后好。
Objective To investigate the clinical manifestations,pathological morphology,treatment and prognosis of follicular lymphoma in children.Methods The clinical data of children with follicular lymphoma treated in our hospital from February 2009 to July 2020 were retrospectively studied,including onset age,clinical manifestation,clinical stage,histological morphology,treatment and prognosis.Results All the 8 children were male,with the median onset age of 6 years and 2 months.The onset sites were lymph nodes in 3 cases,extranodal in 5 cases,stageⅠin 1 case,stageⅡin 3 cases,stageⅢin 4 cases;histological grade in 3 cases of children type,common type in 5 cases,were treated with chemotherapy,of which 5 cases were combined with rituximab treatment,good prognosis was achieved.Conclusions Follicular lymphoma in children is different from that in adults.It is more common in males and has no bone marrow infiltration.It has its own pathological morphology and immunophenotype.The clinical stage is mainly stageⅠ-Ⅲ,and the pathological grade is mainly gradeⅢ.Prognosis is good.
