摘要
目的探讨影响产妇泌乳Ⅱ期乳汁分泌的相关因素,改善产妇泌乳Ⅱ期乳汁的质量,提高纯母乳喂养率。方法采用前瞻性抽样调查法,选取2019年6月1-30日在该院足月分娩、无妊娠合并症的产妇和新生儿332例。采用自行设置的喂哺记录单,收集并记录产妇的基本信息、产后早吸吮时间和时长、产后前2 d的喂哺情况等,并利用SPSS 22.0统计学软件对所得数据进行统计学分析。结果早吸吮时间为(4.24±5.39)h,吸吮时长为(11.96±8.73)min;产后24 h喂哺次数为(2.80±2.18)次,喂哺时间为(11.39±9.23)min,添加配方奶次数为(1.58±1.70)次;产后48 h喂哺次数为(5.86±3.263)次,喂哺时间为(16.89±10.61)次,添加配方奶次数为(4.21±3.16)次。孕期是否参加母乳讲座和婴儿早期含接的培训对于产后顺利进行早吸吮有显著影响(均P<0.05);产后前2 d喂哺次数和喂哺时长对后继能否做到纯母乳喂养也有显著影响(均P<0.05)。结论泌乳Ⅱ期乳汁分泌延迟或继发性泌乳与孕期是否参加母乳讲座和婴儿早期含接的培训相关,也与产后前2 d喂哺次数和喂哺时长相关,做好个性化的母乳讲座和实践培训,可改善产妇泌乳Ⅱ期乳汁的质量,提高纯母乳喂养率。
Objective To explore the related factors of milk secretion during the second stage of lactation period,improve milk quality of postpartum women,and increase exclusive breastfeeding rate.Methods A prospective sampling survey was performed,332 postpartum women(without pregnancy complications)and neonates(full-term delivery)were selected from June 1 st to June 30 th in 2019 from the Third Affiliated Hospital of Guangzhou Medical University.A self-designed lactation record was used to collect basic information of the postpartum women,postpartum early sucking time and duration time,lactation status within two days after birth of the neonates.SPSS 22.0 software was used to analyze the data statistically.Results The postpartum early sucking time was(4.24±5.39)hours,the duration time of postpartum early sucking was(11.96±8.73)minutes,the times of lactation within 24 hours after birth was(2.80±2.18),the lactation time was(11.39±9.23)minutes,the times of adding formula milk was(1.58±1.70);the times of lactation within 48 hours after birth was(5.86±3.263),the lactation time was(16.89±10.61)minutes,the times of adding formula milk was(4.21±3.16);participating in breast milk lectures and infantile early sucking training had significant impact on successful early sucking after birth(P<0.05);the times of lactation within 48 hours after birth and duration time were influencing factors of exclusive breastfeeding(P<0.05).Conclusion Delayed milk secretion or secondary lactation during the second stage of lactation period is correlated with participating in breast milk lectures and infantile early sucking training or not,which is also correlated with the times of lactation within 48 hours after birth and duration time of lactation,personalized breast milk lectures and practical training can improve milk quality during the second stage of lactation period and exclusive breastfeeding rate.
