早期强化本体感觉训练对前交叉韧带重建术后下肢运动控制的影响 被引量:20

Effects of early intensive proprioceptive training on the motor control of lower limb after anterior cruci ligament reconstruction surgery

摘要 目的:探讨早期强化本体感觉训练对前交叉韧带重建术后下肢运动控制的影响。方法:收集2018年9月至2020年8月在本院康复科接受康复治疗的65例膝关节前交叉韧带损伤重建术后患者,随机分为治疗组(n=32)和对照组(n=33),两组患者重建术后均进行常规运动康复训练。术后第4周起,对照组继续给予常规康复训练,治疗组在常规康复训练的基础上,另外接受强化本体感觉训练治疗,20min/次,1次/日,5日/周。比较两组患者术后第4周、第6周和第8周随访时膝关节Lysholm评分、Holden步行功能评分及动态平衡仪本体感觉评分的差异。结果:术后第4周和第6周随访,两组患者各项评分比较,差异无统计学意义(P>0.05)。术后第8周随访,治疗组的膝关节Lysholm评分、Holden步行功能评分及动态平衡仪本体感觉评分分别为67.99±9.23、4.22±0.79、0.21±0.07,对照组分别为62.88±9.68、3.82±0.77、0.25±0.07,两组患者各项评分与第4周评分比较,差异有统计学意义(P<0.05);组间评分比较,治疗组优于对照组(P<0.05)。结论:早期强化本体感觉训练能明显改善膝关节前交叉韧带重建术后患者下肢运动控制能力;且随治疗时间延长,其疗效更为明显。 Objective To explore the effects of early intensive proprioceptive training on the motor control of lower limb after anterior cruci ligament(ACL)reconstruction surgery.Methods Sixty-five patients with ACL reconstruction in our hospital from September 2018 to August 2020 were divided into treatment group(n=32)and control group(n=33)randomly.All patients were treated with routine rehabilitation training after ACL reconstruction.Since the 4th week after ACL reconstruction,while the treatment group were received intensive proprioceptive training,lasting for 20 minutes,once a day,and 5 days per week.The Lysholm knee function score,Holden classification score and the pre-intervention and post-intervention average trace error were measured as main index of clinical effects at the 4th,the 6th,the 8th week.Results The 4th and the 6th week after ACL reconstruction,there were no significant difference in the score of all measure(P>0.05).The 8th week after ACL reconstruction,the Lysholm score,Holden score and score in the treatment group were 67.99±9.23,4.22±0.79,0.21±0.07,respectively,and those scores in the control group were 62.88±9.68,3.82±0.77,0.25±0.07,respectively.Comparing with the scores,there were significant differences in both groups between the 4th and the 8th week after ACL reconstruction(P<0.05),and the treatment group were better than the control group(P<0.05).Conclusion The early intensive proprioceptive training may significantly improve the motor control of lower limb functions in patients with ACL reconstruction,with the extension of the intensive proprioceptive training time,its effect will be more obvious.
