摘要
目的总结儿童毛发上皮瘤在反射式共聚焦显微镜(RCM)下图像特征,探讨RCM的诊断价值。方法选择临床拟诊为毛发上皮瘤患儿15例,行RCM及组织病理学检查。结果经RCM及组织病理学检查证实,15例患儿中毛发上皮瘤13例,传染性软疣1例,汗管瘤1例。13例确诊为毛发上皮瘤患儿RCM表现:表皮大致正常,真皮浅层见瘤体团块,中央见低折光蛙卵状细胞样结构(对应基底样细胞),周围见高折光纤维结构(对应纤维组织)。其中3例还可见圆形高折光物质(对应角囊肿),外周环绕明暗相间结构(对应梭形角质形成细胞)。结论毛发上皮瘤患儿RCM图像有特征性,与组织病理学表现有较好的一致性,其可作为有效的辅助诊断手段。
Objective To summarize the image characteristics of trichoepithelioma in children under reflective confocal microscopy(RCM)and to evaluate the diagnostic value of RCM for the disease.Methods RCM and histopathologic examination were performed in 15 children with clinically suspected trichoepithelioma.Results Thirteen cases were confirmed as trichoepithelioma by histopathologic examination,as well as one case as molluscum contagiosum and one case as syringoma.The RCM manifestations of 13 children with trichoepithelioma were as follows:epidermis was generally normal,lumps of tumor were observed in the superficial layer of dermis,with low-refractive index frog oocyte like structure(corresponding to basal cells)located in the central area and high-refractive index fibrous structure(corresponding to fibrous tissue)in the periphery.Besides,round high-refractive index materials(corresponding to the keratocysts)surrounded by the structure of alternating light and dark(corresponding to the spindle keratinocytes)were found in 3 cases.Conclusion Trichoepithelioma in children have characteristic images under RCM,which is consistent with the histopathologic findings,so RCM examination can be used as an auxiliary diagnostic method for children with trichoepithelioma.
