摘要
目的分析围绝经期综合征患者中医证候分布及发病影响因素。方法选择2015年7月-2019年7月在江西中医药大学附属医院接受常规检查的围绝经期女性674例为研究对象,其中200例围绝经期综合征患者为研究组,其他474例非围绝经期综合征女性中,随机选取200例围绝经期女性为对照组。判断患者证候,分析围绝经期综合征患者发病的影响因素。结果围绝经期综合征患者的主要临床症状为烘热汗出(81.50%)、腰膝酸软(76.00%)、手足心热(64.00%)、头晕耳鸣(60.50%)等;主要经带情况为月经先期(48.50%),带下量多(42.50%),经色黯、夹血块(36.00%)等;主要舌脉情况为苔少(23.50%)、舌红(23.00%)、舌边有瘀斑瘀点(19.00%)等;主要中医证候为肾阴虚证(38.00%)、肾阴阳两虚证(14.00%)、血瘀证(12.00%)等。多因素Logistic回归分析结果显示:文化程度小学及以下、下岗/退休、产次>2次是围绝经期综合征发病的影响因素(均OR>1,均P<0.05)。结论围绝经期综合征发病机制复杂,受多种因素影响,患者中医证候分布广泛,症状各异,应针对女性早期可能的围绝经期综合征影响因素实施有效干预,对于确诊的围绝经期综合征患者,应根据患者不同症状判断其证候,进行辨证施治。
Objective To analyze the distribution of syndromes of traditional Chinese medicine in patients with perimenopausal syndrome and influencing factors. Methods A total of 674 perimenopausal women receiving routine examination in Affiliated Hospital of Jiangxi University of Chinese Medicine from July 2015 to July 2019 were selected as study object,among 674 perimenopausal women,200 perimenopausal women with perimenopausal syndrome were designed as study group,among the other 474 perimenopausal women without perimenopausal syndrome,200 perimenopausal women were randomly selected as control group. The syndromes of traditional Chinese medicine were determined,the influencing factors of perimenopausal syndrome were analyzed. Results The main clinical symptoms of patients with perimenopausal syndrome were fever and sweating( 81. 50%),soreness and weakness of waist and knees( 76. 00%),feverishness in palms and soles( 64. 00%), dizziness and tinnitus( 60. 50%);the main menstrual and leucorrheal symptoms were advanced menstruation( 48. 50%),much leucorrhea( 42. 50%),dark menstrual color,blood clots( 36. 00%);main tongue symptoms were less furred tongue( 23. 50%),red tongue( 23. 00%),tongue petechiae and ecchymoses( 19. 00%);main syndromes of traditional Chinese medicine were kidney-Yin deficiency( 38. 00%),deficiency of kidney Yin and Yang( 14. 00%),and blood stasis( 12. 00%). Multivariate Logistic regression analysis showed that educational level of primary school or below,out of work/retirement,and parity>2 were influencing factors of perimenopausal syndrome( OR>1,P<0. 05). Conclusion The pathogenesis of perimenopausal syndrome is complicated,which is affected by many factors,the distribution of syndromes of traditional Chinese medicine is wide,the symptoms are different,effective intervention should be taken according to the possible influencing factors of perimenopausal syndrome;for the patients diagnosed as perimenopausal syndrome definitely,the syndromes of traditional Chinese medicine should be determined according to symptoms,diagnosis and treatment based on an overall analysis of the illness and the patient’s condition should be performed.
