摘要
目的探讨专科护士主导的多学科团队(MDT)护理模式在妊娠期糖尿病(GDM)患者中的应用效果。方法采用随机数字表法将400例GDM患者分为常规组和专科组,各200例。常规组围生期接受常规用药、饮食、运动和心理指导等综合干预;专科组在常规组基础上给予专科护士指导的MDT护理干预。比较两组的血糖指标、焦虑情绪、自我管理行为及妊娠结局。结果干预后,两组的FPG、PBG水平和HbA1c均降低,且专科组低于常规组(P<0.01)。干预后,两组的SAS评分均降低,自我管理行为能力评分均升高,且专科组优于常规组(P<0.01)。专科组的羊水过多、胎膜早破、剖宫产、早产、巨大儿、低血糖发生率均明显低于常规组(P<0.05)。结论在GDM患者中开展专科护士主导的MDT护理干预,有利于血糖控制,消除患者焦虑情绪,增强其自我管理行为能力,减少不良母婴结局发生。
Objective To explore the application effect of multidisciplinary team(MDT)nursing mode led by specialist nurses in patients with gestational diabetes mellitus(GDM).Methods Four hundred patients with GDM were divided into routine group and specialized group by random number table method,with 200 cases in each group.The routine group received comprehensive intervention such as routine medication,diet,exercise and psychological guidance during perinatal period,and the specialized group was given MDT nursing intervention led by specialized nurses on the basis of the routine group.The blood glucose,anxiety,self-management behavior and pregnancy outcome were compared between the two groups.Results After intervention,the levels of FPG,PBG and HbA1c in the two groups decreased,and those in the specialist group were lower than the routine group(P<0.01).After intervention,the SAS score of both groups decreased,the self-management behavior ability score of both groups increased,and those in the specialist group were better than the routine group(P<0.01).The incidences of polyhydramnios,premature rupture of membranes,cesarean section,premature delivery,macrosomia and hypoglycemia in the specialist group were significantly lower than those in the routine group(P<0.05).Conclusion MDT nursing intervention led by specialized nurses in GDM patients is conducive to blood glucose control,eliminating anxiety,enhancing ability of self-management behavior and reducing the occurrence of adverse maternal and infant outcomes.
