摘要
目的提高医院的临床合理用药水平。方法随机抽取某院2016年至2019年的22073张门诊中药饮片处方、13178份住院医嘱,点评其合理性,分析不合理原因,并提出改进措施,观察改善效果。结果22073张中药饮片处方中,不合理处方共430张(1.95%),包括不规范处方353张(1.60%)、用药不适宜处方77张(0.35%);13178份医嘱中,不合理医嘱共1003份(7.61%)。实施干预措施后,中药饮片处方不合理率由2016年的7.57%降至2019年的0.74%;医嘱不合理率由2016年的8.94%降至2019年的8.13%。结论该院中药饮片处方/医嘱的不合理现象2016年至2019年逐渐减少;对中药饮片处方/医嘱进行点评具有必要性及重要性。
Objective To improve the rational use of drugs in the hospital.Methods A total of 22073 outpatient prescriptions of Chinese herbal pieces and 13178 medical orders for inpatients in a hospital were randomly selected from 2016 to 2019.The rationality of the prescriptions was evaluated,the unreasonable reasons were analyzed,and the improvement measures were put forward to observe the improvement effect.Results Among 22073 prescriptions of Chinese herbal pieces,430 prescriptions(1.95%)were irrational,including353 non-standard prescriptions(1.60%)and 77 inappropriate prescriptions(0.35%).Among 13178 medical orders,1003 medical orders(7.61%)were irrational.After the implementation of the intervention measures,the irrational rate of Chinese herbal pieces prescription decreased from 7.57%in 2016 to 0.74%in 2019.The irrational rate of medical orders decreased from 8.94%in 2016 to8.13%in 2019.Conclusion The irrational phenomenon of Chinese herbal pieces prescription/medical orders in our hospital gradually decreased from 2016 to 2019.It is necessary and important to comment on the prescriptions/medical orders of Chinese herbal pieces.
