摘要
目的探讨核磁共振(MRI)、18氟-氟代脱氧葡萄糖(18 F-FDG)正电子发射断层扫描成像/计算机断层扫描成像(PET/CT)在感染性脊柱炎诊断中的应用价值。方法回顾2001年1月~2019年12月期间在河北北方学院附属第一医院进行感染性脊柱炎筛查的100例患者的临床资料。所有受试者经组织病理学和细菌学检查,有明确的诊断结果,并接受MRI及18 F-FDG PET/CT检查。分析MRI、18 F-FDG PET/CT检查在感染性脊柱炎诊断中的意义。结果MRI、18 F-FDG PET/CT检查均为感染性脊柱炎诊断结果的独立影响因素。ROC曲线分析结果显示,MRI、18 F-FDG PET/CT及联合预测因子在诊断感染性脊柱炎中的曲线下面积(AUC)分别为0.896、0.958、0.973,均有统计学意义(P<0.01)。MRI与18 F-FDG PET/CT联合检测在感染性脊柱炎诊断中的诊断效能最理想,准确度、敏感度、特异度分别为99.00%、100.00%、97.50%。结论将MRI与18 F-FDG PET/CT联合检测感染性脊柱炎,能够明显提升诊断的准确率。
Objective To explore the application value of MRI and PET in diagnosis of infectious spondylitis.Methods The clinical data of 100 patients screened for infectious spondylitis at the First Affiliated Hospital of Hebei North University from January 2001 to December 2019 were reviewed.All subjects underwent histopathological and bacteriological examinations and were positively diagnosed.MRI and 18 F-FDG PET/CT were performed.The significance of MRI and 18 F-FDG PET/CT in the diagnosis of infectious spondylitis was analyzed.Results MRI and 18 F-FDG PET/CT were independent factors affecting the diagnosis of infectious spondylitis.The ROC curve analysis showed that the AUC values of MRI,18 F-FDG PET/CT and the combined predictors in the diagnosis of infectious spondylitis were 0.896,0.958 and 0.973,respectively(P<0.01).The combined detection of MRI and 18 F-FDG PET/CT was the most ideal method in the diagnosis of infectious spondylitis,with the accuracy,sensitivity and specificity of 99.00%,100.00%and 97.50%,respectively.Conclusion The combined detection of MRI and 18 F-FDG PET/CT in the diagnosis of infectious spondylitis can significantly improve the diagnostic accuracy.
