摘要
目的探讨声触诊弹性成像(sound touch elastography,STE)在慢性乙型肝炎患者肝纤维化评估中的应用价值。方法对95例慢性乙型肝炎患者均行STE及血清学检查,以病理结果为金标准,分析STE弹性模量值及血清学检查与肝纤维化病理分期的相关性,评估STE检查诊断肝纤维化受试者工作特征(ROC)曲线下的面积,评价STE在肝纤维化各分期的诊断价值。结果单因素方差分析、多组间两两比较(采用LSD法)结果显示,年龄、STE、纤维化-4指数(FIB-4)、γ-谷氨酰转肽酶(GGT)与肝纤维化病理分期存在统计学差异(P<0.05);Spearman秩相关分析显示,STE、天冬氨酸转氨酶/血小板比值指数(APRI)、FIB-4与肝脏纤维化分期呈正相关(P<0.01);绘制STE、FIB-4、APRI诊断肝纤维化≥S1期ROC曲线下面积分别为:0.853、0.651、0.594;绘制STE诊断≥S1期、≥S2期、≥S3期ROC曲线下面积分别为0.853、0.802、0.989。结论STE在评估慢性乙型肝炎患者的肝纤维化程度中有较好的应用价值。
Objective To explore the application value of Sound Touch Elastography(STE)in the evaluation of liver fibrosis in chronic hepatitis B patients.Methods A total of 95 chronic hepatitis B patients were selected for research subjects.The elastic modulus of STE and serological examination were performed on all patients.Finally,the pathological result was used as the gold standard.The relativity was analyzed betweenSTE,serological examination and liver fibrosis stages.The area under ROC curve of STE was used to evaluate the diagnosis of liver fibrosis stages,which couldjudge the diagnostic efficacy of STE.Results Age,STE,FIB-4(the fibrosis score 4),GGT(γ-glutamyl transpeptidase)were significant differences among the pathological staging groups(P<0.05);Spearman correlation analysis showed that STE,APRI(aminotransferase to platelet ratio index),FIB-4were positively correlated with the pathological staging groups(P<0.01).The areas under ROC curve of STE,FIB-4 and APRI in diagnosis of liver fibrosis inthe≥S1 stage were 0.853,0.651 and 0.594 respectively.The areas under ROC curve of≥S1stage,≥S2stage,and≥S3stage were 0.853,0.802 and 0.989 respectively.Conclutions STE is a promising technique for assessing liver fibrosis in chronic hepatitis B patients.
