摘要
目的:探讨腹腔镜肾部分切除术(LPN)中缝线长度与肿瘤体积、外凸率的相关性。方法:收集2019年11月至2020年7月行LPN的36例患者的临床资料,统计肾肿瘤体积、外凸率等肿瘤指标及术中缝合层数、每层缝线长度、缝线总长度等。使用SPSS软件对数据进行相关性及回归分析。结果:LPN术中缝线长度与肿瘤体积和(或)外凸率存在相关性,可通过与肿瘤体积、肿瘤外凸率之间的关系估算得出。缝线长度与肿瘤体积和(或)肿瘤外凸率存在线性关系时,可得出缝线长度计算公式,即缝线长度=肿瘤体积×a+肿瘤外凸率×b+c(a、b均为非标准化系数,P<0.05;c为常量,P<0.05)。两层缝合或不考虑缝合层数时,缝线总长度与肿瘤体积及外凸率之间的线性拟合度最好,两层缝合缝线总长度=肿瘤体积×0.5-外凸率×6+40(P<0.05,R2=0.847);不考虑缝合层数时,缝线总长度=肿瘤体积×0.7-外凸率×9+36(P<0.05,R2=0.772)。结论:LPN术中可通过肿瘤体积、外凸率等信息提前估算出术中所需缝线长度,从而为术者提供适宜的缝线。
Objective:To explore correlation of suture length with the tumor volume and exophytic properties in laparoscopic partial nephrectomy.Methods:The clinical data of patients undergoing laparoscopic partial nephrectomy from Nov.2019 to Jul.2020 were collected.And various tumor indicators such as renal tumor volume,exophytic properties,intraoperative suture layers,suture length of each layer and total length of suture were counted.SPSS software was used for correlation and regression analysis.Results:In laparoscopic partial nephrectomy,suture length was associated with tumor volume and/or exophytic properties,it could be estimated from the relationship between the tumor volume and exophytic properties.When the layer number of sutures were different,the relationship of the length of sutures required during the operation with tumor volume and exophytic properties was different.When there was a linear relationship between the length of sutures and tumor volume and/or exophytic properties,the formula for calculating the length of sutures could be obtained,namely suture length=tumor volume×a+exophytic properties×b+c(both a and b were non-standardized coefficients,P<0.05,c was constant,P<0.05).In the case of two layers of sutures or the number of suture layers was not considered,the linear fitting degree of the total suture length with tumor volume and exophytic properties was the best.Suture length(two-layer sutures)=tumor volume×0.5-exophytic properties×6+40(P<0.05,R2=0.847).Suture length(number of suture layers not considered)=tumor volume×0.7-exophytic properties×9+36(P<0.05,R2=0.772).Conclusions:In laparoscopic partial nephrectomy,the length of the intraoperative suture can be estimated in advance based on the tumor volume and exophytic properties,so as to provide the surgeon with the perfect suture.
