摘要
目的分析超声对甲状腺结节良恶性的鉴别诊断价值。方法选择本院2019年1月至2019年12月收治的甲状腺结节良性患者35例,纳入良性组;另选2019年1月至2019年12月收治的甲状腺结节恶性患者35例,纳入恶性组。采用彩色多普勒超声诊断仪检查。比较两组患者二维灰阶超声图像特征、结节钙化情况、血流情况,分析其诊断价值。结果恶性组患者边缘模糊、无包膜、宽声晕、低回声、后方衰减、颈部淋巴结肿大率高于良性组(P<0.05)。恶性组患者微钙化率高于良性组(P<0.05),粗钙化率、弧状钙化率低于良性组(P<0.05)。恶性组收缩期峰值血流速度、阻力指数高于良性组(P<0.05),舒张末期血流速度小于良性组(P<0.05)。结论彩色多普勒超声在甲状腺结节良恶性鉴别诊断上有价值,能提升诊断率,有利于临床治疗。
Objective To investigate the value of ultrasound in the differential diagnosis of benign and malignant thyroid nodules.Methods From January 2019 to December 2019,35 patients with benign thyroid nodule(benign group)and 35 patients with malignant thyroid nodule(malignant group)were selected.All patients underwent color Doppler ultrasound examination.The characteristics of two-dimensional grayscale ultrasound images,nodular calcification and blood flow were compared between the two groups,and their diagnostic value was also verified.Results The rate of nodules with blurred edges,no envelope,wide acoustic halo,low echo,posterior attenuation,and cervical lymphadenopathy were higher in malignant group than those in benign group(P<0.05).The rate of micro calcification in malignant group was higher than that in benign group(P<0.05).The rate of macro calcifications and circumjacent arc calcifications in malignant group were lower than those in benign group(P<0.05).The peak systolic blood velocity and resistance index of the malignant group were higher than those of the benign group(P<0.05),while the end-diastolic blood velocity of the malignant group was lower than that of the benign group(P<0.05).Conclusion Color Doppler ultrasound is valuable in the differential diagnosis of benign and malignant thyroid nodules,can improve the diagnosis rate,and is conducive to clinical treatment.
