摘要
目的调查并分析老年女性患者尿路感染细菌的分布情况及对常用药物的耐药性,探究患者病情转归,为临床治疗提供参考。方法选取2017年至2019年首都医科大学附属北京朝阳医院西院收治的≥60岁的老年女性尿路感染患者354例,对分离出的菌株进行鉴定和药敏试验,分析病原菌对常用抗菌药物的耐药性,以及患者病情转归情况。结果354株分离菌株中以革兰阴性菌为主,占比为71.8%,其中以大肠埃希菌最为常见,其次为肺炎克雷伯菌、奇异变形菌和铜绿假单胞菌。革兰阳性菌占比25.4%,以肠球菌属为主。另外,真菌感染的数量明显增加,占比2.8%。病情转归方面,治疗结束后224例(63.27%)患者彻底康复,86例患者(24.29%)转为慢性,41例(11.58%)患者发展为阴道炎,3例(0.86%)发展为肾炎。结论老年女性患者尿路感染病原菌种类较多,不同病原菌对抗菌药物表现出不同的耐药性,在临床治疗中应根据药敏试验结果给予合理抗菌药物治疗,提高临床抗感染治疗的效果。
Objective To investigate and analyze the distribution and resistance to commonly used drugs of urinary tract infection bacteria in elderly female patients.Methods From 2017 to 2019,354 elderly female patients with urinary tract infection aged 60 years or older,who were admitted to the West Hospital of Beijing Chaoyang Hospital,Capital Medical University,were selected as the study subjects.The isolated strains were identified and drug susceptibility tests were conducted to analyze the resistance of pathogens to commonly used antibacterial drugs,and related patient’s outcomes.Results Gram-negative bacteria accounted for 71.8%of the 354 isolates.Among them,Escherichia coli was the most common one,followed by Klebsiella pneumoniae,Proteus mirabilis and Pseudomonas aeruginosa.Gram-positive bacteria accounted for 25.4%,mainly Enterococcus species.In addition,the number of fungal infections increased significantly,accounting for 2.8%.In terms of disease outcome,224 patients(63.27%)recovered completely after the treatment,86 patients(24.29%)became chronic,41 patients(11.58%)developed vaginitis,and 3 patients(0.86%)developed nephritis.Conclusion There are multiple types of pathogenic bacteria in urinary tract infection in elderly female patients,and different pathogens show different resistance to antibacterial drugs.In clinical treatment,a reasonable antimicrobial treatment strategy should be provided according to the results of drug sensitivity test to improve the effect of clinical anti-infection treatment.
