基于跨理论模型的健康教育对宫颈癌患者自我效能及心理弹性的影响 被引量:24

Effects of health education based on a cross-theoretical model on self-efficacy and psychological resilience in patients with cervical cancer

摘要 目的探讨基于跨理论模型的健康教育对宫颈癌患者自我效能及心理弹性的影响。方法选取2018年4月至2020年10月间西电集团医院收治的114例宫颈癌患者作为研究对象。根据随机信封抽签原则把患者分为观察组与对照组,每组57例。对照组患者给予常规健康教育,观察组患者给予基于跨理论模型的健康教育,两组患者护理观察时间为1个月,采用一般自我效能感量表和心理弹性量表评价患者自我效能及心理弹性的变化情况。结果观察组的术后首次肛门排气时间、进流食时间、下地活动时间与住院时间都少于对照组,差异均有统计学意义(均P<0.05)。两组患者护理后的自我效能评分高于护理前,观察组高于对照组,差异均有统计学意义(均P<0.05)。两组患者护理后的坚韧和乐观等心理弹性评分高于护理前,观察组高于对照组,差异均有统计学意义(均P<0.05)。护理后,观察组的躯体、角色、情感、认知和社会等生活质量评分均高于对照组,差异均有统计学意义(均P<0.05)。结论基于跨理论模型的健康教育应用于宫颈癌患者中,能改善自我效能和心理弹性状况,利于促进患者康复,提高生活质量。 Objective To explore the effects of health education based on a cross-theoretical model on self-efficacy and psychological resilience in cervical cancer patients.Methods From April 2018 to October 2020,a total of 114 cervical cancer patients diagnosed and treated at Xian XD Group Hospital were selected.The patients were divided into an observation group and a control group with 57 patients in each group using the principle of random envelope drawing.The observation group were given health education based on a cross-theoretical model and the control group were given conventional health education.The two groups were followed-up for one month.The general self-efficacy scale and resilience scale were used to assess changes in self-efficacy and resilience.Results The time to postoperative anal exhaust,time to fluid feeding,time to out-of-bed activity and length of hospital stay were shorter in the observation group than in the control group(all P<0.05).The self-efficacy scores were increased in the two groups after nursing(all P<0.05),and the scores were higher in the observation group than in the control group(all P<0.05).The mental resilience scores of tenacity and optimism were elevated in the two groups after nursing compared with before nursing(all P<0.05),and the scores were higher in the observation group than in the control group(all P<0.05).After nursing,the scores of quality of life in aspects of physical body,role,emotion,cognition,and society were higher in the observation group than in the control group(all P<0.05).Conclusion Health education based on a cross-theoretical model can improve self-efficacy and psychological resilience,promote the recovery,and improve quality of life in patients with cervical cancer.
