摘要
目的探讨坤泰胶囊联合马来酸氟伏沙明治疗围绝经期失眠且伴有抑郁的临床效果。方法选取2018年7月至2019年10月新乡医学院第二附属医院收治的96例围绝经期失眠且伴有抑郁患者,根据治疗方法不同分为对照组和试验组,每组各48例。对照组口服马来酸氟伏沙明片进行治疗,试验组口服马来酸氟伏沙明联合坤泰胶囊进行治疗,治疗4周后,采用汉密尔顿抑郁量表(HAMD)、匹兹堡睡眠治疗指数表(PSQI)、Kupperman量表评定临床疗效。采用不良反应症状量表(TESS)评定不良反应。结果试验组患者的总有效率和基本痊愈率均显著高于对照组,差异有统计学意义(P<0.05);两组治疗后HAMD、PSQI、Kupperman量表评分较治疗前均显著降低,差异有统计学意义(P<0.05);且试验组治疗后HAMD、PSQI、Kupperman量表下降程度大于对照组,差异有统计学意义(P<0.05);两组TESS评分比较,差异无统计学意义(P>0.05)。结论坤泰胶囊联合马来酸氟伏沙明治疗围绝经期失眠且伴有抑郁的疗效显著,可缓解患者的抑郁症状,提高睡眠质量和生活质量。
Objective To investigate the clinical efficacy of Kuntai Capsule combined with fluvoxamine maleate on perimenopausal insomnia complicated with depression.Methods A total of 96 patients with perimenopausal insomnia complicated with depression admitted to the Second Affiliated Hospital of Xinxiang Medical University from July 2018 to October 2019 were selected and divided into the control group(n=48)and the experimental group(n=48)according to different treatment methods.The control group was treated orally with fluvoxamine maleate tablets,while the experimental group was treated orally with fluvoxamine maleate combined with Kuntai Capsule.After 4 weeks of treatment,the clinical efficacy was evaluated with Hamilton depression scale(HAMD),Pittsburgh sleeps therapeutic index(PSQI)scale and Kupperman scale.Adverse reactions(ADRs)were evaluated by treatment emergent symptom scale(TESS).Results The total efficacy rate and basic recovery rate of patients in the experimental group were both significantly higher than those in the control group,with statistically significant differences(P<0.05).After treatment,the scores of HAMD,PSQI and Kupperman scale in the two groups were both significantly lower than those before treatment,with statistically significant differences(P<0.05).Meanwhile,after treatment,the scores of HAMD,PSQI and Kupperman scale in the experimental group decreased more than those in the control group,with statistically significant differences(P<0.05).There was no statistically significant difference in TESS scores between the two groups(P>0.05).Conclusion Kuntai Capsule combined with fluvoxamine maleate is effective in treating perimenopausal insomnia complicated with depression,and it can alleviate the depressive symptoms and improve the qualities of sleep and life.
