摘要
目的探讨垂直双锁定加压钢板治疗对肱骨干下段骨折患者手术指标、肘关节Mayo功能评分及并发症的影响。方法选取2016年1月—2019年1月在本院行肱骨干下段骨折手术治疗的80例患者,据手术方法不同分为观察组(采用垂直双锁定加压钢板治疗)和对照组(采用传统单锁定加压钢板治疗),每组40例。对比2组临床疗效、手术相关指标,分别采用肘关节Mayo功能评分、世界卫生组织生活质量测量量表(WHOQOL-BREF)评分评估2组手术前后肘关节功能及生活质量,术后随访6个月了解2组并发症发生情况。结果观察组优良率明显高于对照组(P<0.05)。观察组手术时间长于对照组,术中出血量少于对照组,住院时间、愈合时间短于对照组,差异有统计学意义(P<0.05或P<0.01)。术后,2组肘关节Mayo功能各项指标评分及总评分均显著升高,且观察组肘关节Mayo功能各项指标评分及总评分均显著高于对照组,差异有统计学意义(P<0.05或P<0.01)。术后,2组WHOQOL-BREF量表社会功能、物质生活、躯体功能、心理功能评分均明显升高,且观察组WHOQOL-BREF量表各指标评分均显著高于对照组,差异有统计学意义(P<0.05或P<0.01)。观察组术后并发症总发生率明显低于对照组(P<0.05)。结论垂直双锁定加压钢板治疗肱骨干下段骨折临床效果好,有利于保护局部神经及血运,并可加速恢复患者肘关节功能,提高患者生活质量,且安全性高。
Objective To investigate effects of vertical double locking compression plate on surgical indexes,Mayo function score for elbow joints and complications in treatment of patients with lower humeral shaft fractures(HSFs).Methods A total of 80 patients receiving surgery for lower HSFs during January 2016 and January 2019 were selected and divided into observation group(treated with vertical double locking compression plate,n=40)and control group(treated with traditional single locking compression plate,n=40)according to different surgical methods.Clinical efficacy and related operation indexes were compared,and elbow joint function and quality of life before and after operation were evaluated by Mayo function score for elbow joints and World Health Organization Quality of Life(WHOQOL)-BREF score respectively,and incidence rates of complications were also observed in the 6 th month of follow-up in two groups.Results The excellent and good rate in observation group was significantly higher than that in control group(P<0.05).In observation group,operative duration was significantly longer,and intraoperative volume of blood loss was significantly less,and length of hospitalization and healing time were significantly shorter than those in control group(P<0.05 or P<0.01).After operation,Mayo function index every scores for elbow joints and the total scores were significantly higher than those before operation in two groups,and the every scores and the total score in observation group were significantly higher than those in control group(P<0.05 or P<0.01).After operation,scores of social function,material life,physical function and psychological function of WHOQOL-BREF were significantly higher than those before operation in two groups,and the scores of each index of WHOQOL-BREF in observation group were significantly higher than those in control group(P<0.05 or P<0.01).The total incidence rate of postoperative complications in observation group was significantly lower than that in control group(P<0.05).Conclusion Vertical double locking compression plate in treatment of patients with lower HSFs may achieve good clinical efficacy,helpfully protect local nerve and blood supply,accelerate the recovery of elbow joint function and improve quality of life with good safety.
