摘要
为了减少立井提升过卷事故中不安全动作的产生,基于事故致因的"2-4"模型,采用归类和统计分析的方法对立井提升过卷事故进行了深入的研究。研究结果表明,导致立井提升过卷事故的发生需要满足5个条件:减速点故障,2 m/s限速开关失效,齐平开关和过卷开关故障,制动机构失灵,缓冲装置不可靠。导致减速点故障的不安全动作为电机转子过电流和超速;导致2 m/s限速开关失效的不安全动作是未及时处理开关故障和编码器计数错误未校正;导致齐平开关和过卷开关故障的不安全动作是未及时处理开关故障和人为操作失误;导致制动机构失灵和缓冲装置不可靠的不安全动作是未完成指定任务和发挥了系统不具备的功能引起,这2类动作发生的次数占共性不安全动作总次数的73.11%,是共性不安全动作培训的重点。
In order to reduce unsafe acts of personnel in overwinding accidents of shaft hosting in coal mine,by applying the accident causation“2-4”model,the overwinding accidents of shaft hosting were studied by statistical analysis and classification.The results showed that overwinding accidents result⁃ing in casualties required to satisfy five conditions:fault of deceleration point,failure of 2 m/s speed limit switch,failure of leveling switch and overwinding switch,failure of brake mechanism and unreliable buffer device.The unsafe action leading to the failure of the deceleration point is the overcurrent and overspeed of the motor rotor.The unsafe action that causes the failure of 2 m/s speed limit switch is that the switch fail⁃ure and the encoder count error are not corrected in time.The unsafe actions leading to the faults of the lev⁃eling switch and the overwinding switch are the failure to deal with the faults of the switch in time and the faults of the manual operation.The unsafe actions that lead to the failure of brake mechanism and the unre⁃liable cushion device are caused by the failure of specified tasks and the function that the system does not have.These two kinds of actions account for 73.11%of the total number of common unsafe actions,which was the focus of the training on the common unsafe acts.
