摘要
目的探讨70°硬性喉镜在咽部异物诊治中的应用价值。方法回顾分析2019年1~6月就诊于我科的405例咽部异物患者的临床资料。患者具有明确的误咽异物史,经详细的口咽部及间接喉镜检查未发现异物,或发现异物但取出困难者随后在局部麻醉下行70°硬性喉镜检查。结果404例(99.8%)患者在硬性喉镜下成功定位异物并顺利取出。异物常见部位依次为舌根154例、扁桃体107例、会厌谷64例、梨状窝46例。53例(13.1%)患者咽部黏膜可见溃疡样物或白膜伴或不伴局部黏膜红肿,对异物具有定位作用。最常见异物为鱼骨(400例),其他少见种类包括:鸡骨、猪骨、蟹壳、瓜子壳、螺钉。结论硬性喉镜具有成像清晰、放大倍数高的特点,对于咽部隐匿性异物的取出具有单人操作、简捷高效、价格低廉、患者耐受程度高的优势。
Objective To investigate the value of 70-degree rigid laryngoscope in the diagnosis and treatment of foreign bodies in the pharynx.Methods A retrospective analysis was performed in 405 patients of foreign bodies in the pharynx who had received 70-degree rigid laryngoscope by local anesthesia at the Department of Otolaryngology,Eye&ENT Hospital,Fudan University from January to June in 2019.Results Four hundred and four cases(99.75%)were successfully removed with the utility of 70-degree rigid laryngoscope.Fish bones(400 cases)were the most frequent types with common sites of tongue base(154 cases),tonsil(107 cases),vallecula(64 cases)and pyriform sinus(46 cases).Ulcers or white lesions with or without swelling of the mucosa were detected in 53 cases(13.1%),which played a landmark effect on the recognition of foreign bodies.Conclusions Rigid laryngoscope has the advantages of offering a clear visualization with high illumination,affording a simple,efficient,affordable and well tolerated way for removal of foreign bodies in the pharynx.
