截肢手术大鼠模型心律失常发生机制研究

摘要 目的探讨截肢手术大鼠模型心律失常的发生机制。方法选取30只健康清洁级雄性SD大鼠,采用随机数字表法将大鼠分为A组、B组、C组、D组和E组,每组各6只,各组大鼠均建立左后肢截肢手术创伤模型,其中不给予A组大鼠抗心律失常药物,分别给予B组、C组、D组和E组大鼠静脉注射利多卡因、普萘洛尔、胺碘酮和维拉帕米,检测并比较各组大鼠手术前后心率、Q-T间期、P-R间期、左心室射血分数、左心室短轴缩短率、肿瘤坏死因子-α、白介素-6和C反应蛋白水平。结果C组、D组和E组大鼠术后15 min、30 min、45 min、60 min、90 min心率均显著低于同期A组和B组(均P<0.05);B组、C组、D组和E组大鼠术后5 min、15 min、30 min、45 min、60 min、90 min和120 min P-R间期均显著长于同期A组(均P<0.05),而Q-T间期均显著短于同期A组(均P<0.05);B组、C组、D组和E组大鼠术后120 min左心室射血分数、左心室短轴缩短率均显著低于同期A组(均P<0.05);A组、B组、C组、D组和E组大鼠术后120 min血清肿瘤坏死因子-α、白介素-6和C反应蛋白水平比较差异均无统计学意义(均P>0.05)。结论截肢手术会导致截肢手术大鼠模型心功能受损,利多卡因、普萘洛尔、胺碘酮和维拉帕米等药物会加重心功能损伤。 Objective To investigate the mechanism of arrhythmia in amputation rat model.Method A total of 30 healthy and clean male SD rats were selected,according to the random number table method,they were divided into group A,group B,group C,group D and group E,with 6 rats in each group,the model of left hindlimb amputation injury was established in each group,group A was not given antiarrhythmic drugs,group B,group C,group D and group E were given intravenous injection of lidocaine,propranolol,amiodarone and verapamil respectively,the heart rate,Q-T interval,P-R interval,left ventricular ejection fraction(LVEF),left ventricular fractional shortening(LVFS),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and C-reactive protein(CRP)were measured and compared before and after in each group.Result The heart rate of group C,group D and group E at 15 min,30 min,45 min,60 min,90 min after surgery were significantly lower than those in group A and group B(all P<0.05);the P-R interval of group B,group C,group D and group E at 5 min,15 min,30 min,45 min,60 min,90 min and 120 min after surgery were longer than those of group A(all P<0.05),and Q-T interval were shorter than those in group A(all P<0.05);the LVEF and LVFS in group B,group C,group D and group E at 120 min after surgery were significantly lower than those in group A(all P<0.05);there were no significant differences in serum TNF-α,IL-6 and CRP between group A,group B,group C,group D and group E at 120 min after surgery(all P>0.05).Conclusion Amputation can lead to impaired cardiac function of the model of left hindlimb amputation injury,and lidocaine,propranolol,amiodarone and verapamil drugs can aggravate impaired cardiac function.
