摘要
目的通过建模关联和聚类分析方法探讨心力衰竭病人中医辨证要素规律。方法收集安徽中医药大学第一附属医院心血管内科2017年6月—2019年12月心力衰竭病人电子病历首程中的中医辨证要素,对其进行规范整理,借助Microsoft Office Excel 2016、SPSS Statistic 24.0、SPSS Modeler 18.0计算机软件建立数据库并进行关联规则分析、聚类分析。结果215例心力衰竭病人中冠心病病人最多(36%),高血压次之(26%)。高频证候要素中症状以心悸、喘促、尿少、喘息、水肿、痰多或泡沫样痰、面色晦暗、胸闷为主;舌象以舌淡黯、舌色淡、舌红少苔、舌有瘀斑瘀点为主;脉象以弦滑、细、涩、沉迟、弱在临床多见;关联分析得到“舌苔and症状→症状”10个,“脉象and症状→症状”12个;聚类分析得到5类重要证型要素分类结局。结论对心力衰竭病人证、症、舌、脉、苔收集并分析,可见气虚血瘀证、阳虚水泛证、气阴两虚证、阳虚血瘀水停证、痰饮阻肺证为心力衰竭临床常见证型。
Objective To explore the rules of TCM syndrome differentiation in patients with heart failure through modeling association and classification cluster analysis.Methods The TCM syndrome differentiation elements of the first electronic medical records of patients with heart failure were collected from June 2017 to December 2019 in the Cardiovascular Medicine Department of First Affiliated Hospital of Anhui University of Chinese Medicine and standardize it.Using Microsoft Office Excel 2016,SPSS Statistic 24.0,SPSS Modeler 18.0 were used to establish a data base to perform association rule analysis and cluster analysis.Results Among the 215 patients with heart failure,coronary heart disease was the most common(36%),followed by hypertension(26%).Among the high frequency syndromes,the main symptoms were palpitation,dyspnea and tachypnea,oliguria,wheezing,edema,Phlegm,dim complexion,chest distress.Tongue coating was mainly gloomy tongue,pale tongue,red tongue,tongue with petechia.Pulse signs were smooth pulse,thready pulse,uneven pulse,slow and sunken pulse,weak pulse.Correlation analysis showed 10"tongue and symptoms→symptoms"and 12"pulse and symptoms→symptoms".Cluster analysis showed the classification outcomes of 5 important syndromes.Conclusion The syndrome of Qi deficiency and blood stasis,Yang deficiency and water flooding,Qi and Yin deficiency,Yang deficiency and water overflowing,Qi-Yin deficiency,Yang deficiency and blood stasis fluid stagnation,phlegm retention obstructing lung were the common types of heart failure.
