摘要
目的探讨改良Hayman缝合术与B-Lynch缝合术对剖宫产术中及产后出血的治疗效果。方法选取2010年1月~2019年6月在我院妇产科进行剖宫产术中出现产后出血行子宫压迫缝合术的产妇81例作为研究对象。根据手术方式不同分为改良Hayman缝合术35例(Hayman组)和B-Lynch缝合术46例(B-Lynch组),观察两组治疗效果。结果Hayman组的手术时间、术中出血量、术后24 h出血量、输血率、术前术后血色素下降程度、产褥病率均小于B-Lynch组,差异有统计学意义(P<0.05);两组有效率、术后ICU入住率、术后住院时间、血性恶露持续时间、晚期产后出血发生率、月经复潮时间比较差异无统计学意义(P>0.05)。结论改良Hayman缝合术相较于B-Lynch缝合术,治疗剖宫产术中及产后出血更为简便快速,可明显缩短手术时间,减少出血量、输血率及产褥病率。
Objective To compare the efficacy of modified Hayman suture and B-Lynch suture for treatment of postpartum hemorrhage in cesarean section.Methods From January 1,2010 to June 30,2019 in China Emergency General Hospital,there were 35 cases with modified Hayman suture(Hayman group)and 46 cases with B-Lynch suture(B-Lynch group)during cesarean section due to intra-operative bleeding.The treatment effects were observed.Results The operation time,amount of postpartum hemorrhage,volume of blood loss 24 hours after surgery,rate of blood transfusion,hemoglobin deceased and puerperal morbidity in the Hayman suture group were less than those of the B-Lynch suture group(P<0.05).There was no statistically significant difference in the efficiency,postoperative ICU occupancy,hospitalization stay,hemorrhagic duration of lochia,incidence of late postpartum hemorrhage and menstruation recovery between the two groups(P>0.05).Conclusion Comparing to B-Lynch suture,modified Hayman suture for the treatment of postpartum hemorrhage in cesarean section is more simple and faster.It can obviously reduce the operation time,bleeding,blood transfusion rate and puerperal morbidity.
