Distribution and drug resistance analysis of wound secretions after open fractures

摘要 目的探讨开放性骨折术后伤口分泌物的常见病原菌分布及耐药情况。方法回顾性统计分析分离的587株病原菌分布及其药敏试验结果。结果革兰阴性杆菌385株(65.6%);革兰阳性球菌202株(34.4%);真菌6株(1.0%)。铜绿假单胞菌及大肠埃希菌对头孢哌酮/舒巴坦和阿米卡星均较敏感,对氨苄西林、头孢唑林和亚胺培南耐药率均>80.0%;鲍氏不动杆菌对左氧氟沙星、头孢哌酮和阿米卡星敏感,对氨苄西林、头孢他啶和庆大霉素耐药率较高。凝固酶阴性葡萄球菌和金黄色葡萄球菌对万古霉素及替考拉宁100.0%敏感,对克林霉素、阿米卡星及左氧氟沙星的耐药率较低,对青霉素、庆大霉素、红霉素、环丙沙星及第一代、二代、三代头孢菌素均明显耐药。结论开放性骨折伤口术后分泌物的病原菌呈多菌株分布,广泛、多重、高度耐药特点,应合理应用抗生素,积极防治伤口感染。 Objective To investigate the distribution of common pathogenic bacteria and drug resistance of wound secretions after open fractures surgery. Methods The distribution of 587 isolated pathogenic bacteria and the results of drug susceptibility test were retrospectively analyzed. Results A total of 385 strains of gram-negative bacilli( 65. 6%),202 strains of gram-positive cocci( 34. 4%),6 strains of fungus( 1. 0%) were collected. Pseudomonas aeruginosa and Escherichia coli were sensitive to cefoperazone/sulbactam and amikacin,and their drug resistance rates to ampicillin,cefazolin and imipenem were greater than 80. 0%. Acinetobacter baumannii was sensitive to levofloxacin,cefoperazone and amikacin,and had higher drug resistance to ampicillin,ceftazidine and gentamicin. Coagulase-negative Staphylococcus aureus and Staphylococcus aureus were 100. 0%sensitive to vancomycin and teicolanine,had low drug resistance to clindamycin,amikacin and levofloxacin,and were significantly resistant to penicillin,gentamicin,erythromycin. Conclusion The pathogenic bacteria secreted after open fracture wound are distributed in multiple strains,which are extensive,multiple and high drug resistant. Therefore,antibiotics should be rationally applied to prevent and treat wound infection.
