S.T.O.N.E评分系统在PCNL术中单/多通道选择中的预测价值 被引量:1

The predictive value of S.T.O.N.E nephrolithometry scoring system in selection of single/multi-channel in percutaneous nephrolithotomy

摘要 目的:探讨S.T.O.N.E评分系统对PCNL术中单通道或者多通道选择的预测价值.方法:回顾性分析116例单侧肾结石的临床资料及治疗方法。根据术中皮肾通道数目分为单通道组(73例)和多通道组(43例)。比较两组患者的年龄、性别、结石侧别、手术时间、S.T.O.N.E评分、结石残留率和并发症,并分析S.T.O.N.E肾结石评分与单/多通道手术方案的预测价值。结果:两组患者年龄、男/女比率、左/右侧比率、结石残留率、并发症发生率比较,差异均无统计学意义(P>0.05);多通道组S.T.O.N.E肾结石评分显著高于单通道组,当评分≥8分时,更加倾向于选择多通道(AUC=0.850,敏感性=0.860,特异性=0.685,P<0.001)。结论:S.T.O.N.E评分系统对于PCNL术中单/多通道手术方案的选择具有一定的预测价值。 Objective:To explore the predictive value of S.T.O.N.E nephrolithometry scoring system for single-channel or multi-channel selection in percutaneous nephrolithotomy (PCNL).Methods:A retrospective analysis of clinical data and treatment methods was conducted which enrolled 116 patients with unilateral kidney stones admitted.According to the number of intraoperative channels,they were divided into single-channel PCNL group (73 cases) and multi-channel PCNL group (43 cases).The age,gender,stone side,operation time,S.T.O.N.E nephrolithometry score,stone residual rate,and complications were compared between the two groups.Results:There were no statistical differences between the two groups on the age,male / female ratio,the left / right ratio,the residual stone rate and the complication rate (all P>0.05).The S.T.O.N.E nephrolithometry score in the multichannel group was significantly higher than that in the single channel group.When the S.T.O.N.E nephrolithometry score≥8,the choice is more preferred multi-channel PCNL (AUC=0.850,Sensitivity=0.860,specificity=0.685,P<0.001).Conclusion:The S.T.O.N.E nephrolithometry scoring system has a certain predictive value for the choice of single-channel or multi-channel in PCN.
