摘要
目的分析不同剂量甲基强的松龙联合丙种球蛋白(IVIG)冲击治疗小儿重症手足口病的临床疗效及安全性。方法选取2019年10月~2020年10月于本院接受治疗的103例重症手足口病患儿作为研究对象,在给予IVIG的基础上,按照激素剂量分为大剂量组(n=43)、小剂量组(n=39)及无激素组(n=21)。比较各组患儿临床指标恢复时间、临床疗效及并发症发生情况。结果小剂量组、大剂量组患儿发热、皮疹、神经系统症状、肺炎表现、白细胞升高及血糖升高恢复时间明显低于无激素组,临床总有效率明显高于无激素组(P<0.05);而小剂量组、大剂量组间比较无差异(P>0.05)。大剂量组患儿治疗期间药物相关不良反应发生率为20.93%,明显高于小剂量组5.13%及无激素组4.76%,差异有统计学意义(P<0.05)。结论小儿重症手足口病患儿使用小剂量甲基强的松龙联合IVIG冲击治疗可快速促进临床症状恢复,临床疗效佳,安全性高,值得临床推广。
Objective This study aimed to analyze the clinical efficacy and safety of intravenous gamma globulin(IVIG) combined with different doses of methylprednisolone pulse therapy in children with severe hand foot mouth disease. Methods A total of 103 children with severe hand-foot-mouth disease(HFMD) admitted to our hospital from October 2019 to October 2020 were selected. According to the inclusion and exclusion criteria, patients were divided into high-dose group(n=43), low-dose group(n=39) and hormone-free group(n=21). The recovery time, clinical efficacy and complications of each group were compared. Results The recovery time of fever, rash, nervous system symptoms, pneumonia,elevation of white blood cell, and elevation of blood sugar in the low-dose group and the high-dose group were lower than those in the hormone-free group(P<0.05), The clinical total effective rate of the low-dose group and the high-dose group was higher than that of the hormone-free group(P<0.05), but there was no difference between the low-dose group and the high-dose group(P>0.05). The incidence of drug-related adverse reactions was 20.93%, 5.13% and 4.76% in the high-dose group, the low-dose group and the hormone-free group, respectively, with significant difference(P<0.05). Conclusion IVIG combined with low dose methylprednisolone pulse therapy can promote the recovery of clinical symptoms in children with severe HFMD, with good clinical efficacy and safety, which is worthy of clinical promotion.
