摘要
目的探讨清肺通腑泻痰汤联合阿奇霉素治疗小儿支原体肺炎(MP)合并肺不张的疗效及安全性。方法选择解放军陆军第八十二集团军医院2018年6月—2020年5月收治的支原体肺炎合并肺不张患儿90例,采用随机数字法分为对照组和观察组各45例。对照组给予阿奇霉素治疗,观察组在对照组用药基础上给予清肺通腑泻痰汤治疗2周。比较2组治疗后症状消失时间、肺复张时间,观察2组治疗前后呼吸频率(RR)、呼气峰流速(PEF)、动脉氧分压[p(O_(2))]、氧合指数(OI),炎症因子[超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)、降钙素原(PCT)]水平变化,统计2组临床疗效及不良反应发生情况。结果观察组治疗后发热、咳嗽、喘息、肺部啰音消失时间和肺复张时间均明显短于对照组(P均<0.05);治疗后,2组RR及血清hs-CRP、IL-6、PCT水平均降低,PEF、p(O_(2))、OI均升高,差异均有统计学意义(P均<0.05);观察组治疗后RR及血清hs-CRP、IL-6、PCT水平均低于对照组(P均<0.05),PEF、p(O_(2))、OI均高于对照组(P均<0.05);观察组总有效率为95.6%(43/45),对照组为80.0%(36/45),观察组明显高于对照组(P<0.05);2组不良反应发生率比较差异无统计学意义(P>0.05)。结论清肺通腑泻痰汤联合阿奇霉素可更有效促进MP合并肺不张患儿症状体征消退及肺复张,改善呼吸功能,减轻炎症反应,临床疗效满意,用药安全。
Objective It is to investigate the efficacy and safety of Qingfei Tongfu Xietan Decoction combined with azithromycin in the treatment of mycoplasma pneumonia(MP)complicated with atelectasis of type of phlegm-heat congesting the lung.Methods A total of 90 children with MP and atelectasis who were admitted to the Hospital of PLA 82nd Group Army from June 2018 to May 2020 were selected and randomly divided into the control group and the observation group,with 45 cases in each group.The control group was treated with azithromycin,and the observation group was treated with Qingfei Tongfu Xietan Decoction on the basis of the control group,both groups were treated for 2 weeks.The symptom disappearance time and lung recruitment time after treatment were compared between the two groups,the changes in respiratory frequency(RR),peak expiratory flow rate(PEF),arterial oxygen pressure[p(O_(2))],oxygenation index(OI),the levels of inflammatory factors[high-sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6),procalcitonin(PCT)]before and after treatment were observed,the clinical efficacy and adverse reactions were evaluated in the two groups.Results After treatment,the disappearance time of fever,cough,wheezing,lung rales,and lung recruitment time of the observation group were significantly shorter than those of the control group(P<0.05);the RR and levels of serum hs-CRP,IL-6,PCT were decreased,PEF,p(O_(2)),OI were increased in the two groups,and the differences were statistically significant(P<0.05);RR and the levels of serum hs-CRP,IL-6,PCT in the observation group after treatment were lower while PEF,p(O_(2)),and OI were all higher than those in the control group(all P<0.05);the total effective rate of the observation group was 95.6%(43/45)and that of the control group was 80.0%(36/45),the observation group was significantly higher than the control group(P<0.05).There was no statistically significant difference in the incidence of reactions between the two groups(P>0.05).Conclusion Qingfei Tongfu Xietan Decoction combined with azithromycin can more effectively promote the regression of symptoms and signs and lung recruitment,improve respiratory function,reduce inflammation,and have satisfactory clinical efficacy with safe medication in children with MP and atelectasis.
