摘要
目的观察益气抗感汤治疗慢性阻塞性肺疾病(COPD)稳定期肺肾气虚证临床疗效及对患者肺功能、免疫功能和生活质量CTA评分的影响。方法本研究采用回顾性研究,选取2018年10月至2019年12月齐齐哈尔市中医医院治疗的96例稳定期肺肾气虚证COPD患者,按照不同治疗方式分为对照组和观察组,每组各48例。对照组患者给予布地奈德福莫特罗粉吸入剂,每次1吸,每日2次;观察组在对照组基础上采用益气抗感汤治疗,每日1剂,分早、晚分服。疗程均为8周。比较两组治疗前后肺功能、T淋巴细胞亚群(CD3^(+)、CD4^(+)、CD8^(+))、生活质量CAT评分变化。结果治疗后两组FEV1、FEV1/FVC均较治疗前升高(P<0.05);与对照组比较,观察组FEV1、FEV1/FVC升高更为显著(P<0.05);两组CD3^(+)、CD4^(+)均较治疗前升高,CD8^(+)较治疗前降低(P<0.05);与对照组比较,观察组CD3^(+)、CD4^(+)升高更显著,CD8^(+)降低更显著(P<0.05);两组CAT评分均较治疗前降低(P<0.05);与对照组比较,观察组CAT评分降低更显著(P<0.05)。结论益气抗感汤对稳定期肺肾气虚证COPD患者有很好的疗效,能够有效的改善患者肺功能及免疫功能,提高生活质量,值得在临床上进一步推广。
Objective To observe the clinical efficacy of Yiqi Kanggan Decoction in treating lung and kidney Qi deficiency syndrome in stable stage of chronic obstructive pulmonary disease(COPD)and its impact on CTA scores of pulmonary function,immune function and quality of life(QoL).Methods In this paper,a retrospective research was conducted.A total of 96 patients of COPD with lung and kidney Qi deficiency syndrome in stable stage admitted to Qiqihar Traditional Chinese Medicine Hospital from October 2018 to December 2019 were selected,and they were divided into the control group(n=48)and the observation group(n=48)according to different treatment methods.Patients in the control group were given budesonide formoterol powder inhalation,one inhalation every time,bid.While on the basis of the control group,the observation group was treated with Yiqi Kanggan Decoction,half dose every time,bid,taken in the morning and evening.The treatment courses were both 8 weeks.The changes of pulmonary function,T lymphocyte subsets(CD3^(+),CD4^(+),CD8^(+))and CAT scores of QoL were compared between the two groups before and after treatment.Results After treatment,FEV1 and FEV1/FVC in both groups were higher than those at baseline(P<0.05).Compared with the control group,the increase of FEV1 and FEV1/FVC in the observation group was more significant(P<0.05).In both groups,levels of CD3^(+)and CD4^(+)were higher than those at baseline,while levels of CD8^(+)were lower than those at baseline(P<0.05).Compared with the control group,the increase of CD3^(+)and CD4^(+)in the observation group was more significant,while the decrease of CD8^(+)was more significant(P<0.05).CAT scores in both groups were lower than those at baseline(P<0.05).Compared with the control group,the CAT scores in the observation group decreased more significantly(P<0.05).Conclusion Yiqi Kanggan Decoction has a good therapeutic efficacy on patients of COPD with lung and kidney qi deficiency in stable stage,which can effectively improve the pulmonary function,immune function and QoL of patients,and is worthy of further promotion in clinical practice.
