慢性心力衰竭患者血清BNP、cTnI、Hcy水平与心功能、室性心律失常的相关性研究 被引量:17

Relationship between serum BNP, cTnI, Hcy levels and cardiac function, ventricular arrhythmia in patients with chronic heart failure

摘要 目的探讨慢性心力衰竭患者血清B型利钠肽(BNP)、心肌肌钙蛋白I(cTnI)、同型半胱氨酸(Hcy)水平与心功能、室性心律失常的相关性。方法选择2018年6月至2019年6月东莞市人民医院80例慢性心力衰竭患者为研究对象,依据美国纽约心脏病学会(NYHA)心功能分级分组,住院期间有无发生室性心律失常分组,同期选择75例门诊健康体检者作为对照组。比较慢性心力衰竭组与对照组、慢性心力衰竭不同心功能分级组、住院期间有无发生室性心律失常组BNP、cTnI、Hcy水平,绘制受试者工作特征曲线(ROC)分析BNP、cTnI、Hcy对室性心律失常的预测价值。结果慢性心力衰竭组BNP、cTnI、Hcy水平高于对照组(P<0.05)。慢性心力衰竭NYHAⅣ级组BNP、cTnI、Hcy水平高于NYHAⅢ级、NYHAⅡ级及NYHAⅠ级组(P<0.05)。慢性心力衰竭室性心律失常组BNP、cTnI、Hcy水平高于非室性心律失常组(P<0.05)。BNP、cTnI、Hcy、BNP+cTnI+Hcy对慢性心力衰竭室性心律失常预测的ROC曲线下面积分别为0.792、0.709、0.690、0.867,灵敏度分别为0.826、0.870、0.765、0.799,特异度分别为0.684、0.561、0.618、0.712。结论慢性心力衰竭患者BNP、cTnI、Hcy水平较健康者明显上升,和心功能有显著相关性,且对室性心律失常的发生有较高预测价值。 Objective To investigate the relationship between the levels of B-type natriuretic peptide(BNP), cardiac troponin I(cTnI), homocysteine(Hcy) in serum and the cardiac function,ventricular arrhythmia in patients with chronic heart failure. Methods Eighty patients with chronic heart failure hospitalized in Dongguan People’s Hospital from June 2018 to June 2019 were selected as the research objects. According to the cardiac function classification of the American New York Heart Association(NYHA), whether ventricular arrhythmia occurred during hospitalization, they were divided into groups. Meanwhile, 75 healthy outpatients were selected as the control group. The levels of BNP, cTnI and Hcy in the chronic heart failure group and the control group, the chronic heart failure group with different cardiac function grades, and the group with and without ventricular arrhythmia during hospitalization were compared, and receiver operating characteristic curve(ROC) was drawn to analyze the predictive value of BNP, cTnI and Hcy on ventricular arrhythmia. Results The levels of BNP, cTnI and Hcy in chronic heart failure group were higher than those in control group(P<0.05). The levels of BNP, cTnI and Hcy in NYHAⅣ group were higher than those in NYHAⅢ, NYHAⅡ and NYHAⅠ group(P<0.05). The levels of BNP, cTnI and Hcy in ventricular arrhythmia group of chronic heart failure were higher than those in non-ventricular arrhythmia group(P<0.05). The areas under ROC curve of BNP, cTnI, Hcy, BNP+cTnI+Hcy for predicting ventricular arrhythmia in chronic heart failure were 0.792, 0.709, 0.690 and 0.867 respectively, the sensitivity were 0.826, 0.870, 0.765 and 0.799, and the specificity were 0.684, 0.561, 0.618 and 0.712.Conclusions The levels of BNP, cTnI and Hcy in patients with chronic heart failure are significantly higher than those in healthy subjects, which is significantly correlated with cardiac function and has high predictive value for the occurrence of ventricular arrhythmia.
