摘要
目的分析导管射频消融术治疗阵发性心房颤动合并快慢综合征的临床效果。方法选择2015年3月至2020年3月我院收治的58例阵发性心房颤动合并快慢综合征患者作为研究对象,采用随机数字表法将其分为参照组和导管射频消融组,各29例。参照组接受常规抗心律失常药物治疗,导管射频消融组在参照组基础上接受导管射频消融术治疗。比较两组的治疗效果。结果治疗3个月后,两组左室舒张末期内径(LVEDD)小于治疗前,平均心率(MEHR)、左室射血分数(LVEF)高于治疗前,且导管射频消融组优于参照组(P<0.05)。治疗3个月后,两组最长RR间期(MRRI)短于治疗前,左房中侧径小于治疗前,且导管射频消融组优于参照组(P<0.05)。治疗3个月后,导管射频消融组症状控制优良率为93.10%,明显高于参照组的68.97%(P<0.05)。治疗12个月后,导管射频消融组房颤复发率为3.45%,明显低于参照组的34.48%(P<0.05)。结论导管射频消融术治疗阵发性心房颤动合并快慢综合征患者效果显著,可明显改善患者的心功能指标,控制症状,降低房颤复发率,部分患者可避免起搏器植入。
Objective To analyze the clinical effect of radiofrequency catheter ablation in the treatment of paroxysmal atrial fibrillation with tachycardia-bradycardia syndrome.Methods From March 2015 to March 2020,58 patients with paroxysmal atrial fibrillation and tachycardia-bradycardia syndrome admitted in our hospital were selected as the research objects,and the patients were divided into reference group and radiofrequency catheter ablation group by random number table method,with 29 cases in each group.The reference group received conventional antiarrhythmic drugs,and the radiofrequency catheter ablation group received radiofrequency catheter ablation on the basis of the reference group.The therapeutic effects of the two groups were compared.Results After 3 months of treatment,the left ventricular end diastolic diameter(LVEDD)in the two groups were smaller than those before treatment,the mean heart rate(MEHR)and left ventricular ejection fraction(LVEF)were higher than those before treatment,and those in the radiofrequency catheter ablation group were better than the reference group(P<0.05).After 3 months of treatment,the maximum RR interval(MRRI)in the two groups were shorter than those before treatment,the left atrium medial diameter were smaller than those before treatment,and those in the radiofrequency catheter ablation group were better than the reference group(P<0.05).After 3 months of treatment,the excellent and good rate of symptom control in the radiofrequency catheter ablation group was 93.10%,which was significantly higher than 68.97% in the reference group(P<0.05).After 12 months of treatment,the recurrence rate of atrial fibrillation in the radiofrequency catheter ablation group was 3.45%,which was significantly lower than 34.48%in the reference group(P<0.05).Conclusion Radiofrequency catheter ablation in the treatment of paroxysmal atrial fibrillation with tachycardia-bradycardia syndrome has a significant effect,which can significantly improve the cardiac function indexes of patients,control symptoms,reduce the recurrence rate of atrial fibrillation,and some patients can avoid pacemaker implantation.
