摘要
目的观察生大黄联合乌司他丁治疗重症急性胰腺炎合并感染临床疗效及对胃肠功能的影响。方法将我院85例重症急性胰腺炎合并感染患者随机分为对照组与观察组。对照组给予乌司他丁治疗,观察组在对照组治疗基础上给予生大黄治疗。比较两组临床疗效;两组治疗前后血浆D-乳酸、内毒素(ET)、二胺氧化酶(DAO)含量及尿乳果糖/甘露醇(L/M)值;两组治疗前后血清肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)和白细胞介素-8(IL-8)含量;比较两组患者腹痛缓解时间、胃肠减压时间及住院时间;统计分析两组患者治疗期间不良反应发生情况。结果观察组治疗总有效率高于对照组(P<0.05);治疗后,观察组血浆D-乳酸、ET、DAO含量及尿L/M值均低于对照组(P<0.05);观察组TNF-α、IL-6、IL-8含量低于对照组(P<0.05);观察组腹痛缓解时间、胃肠减压时间及住院时间均短于对照组(P<0.05);观察组治疗期间不良反应发生率低于对照组(P<0.05)。结论大黄联合乌司他丁治疗重症急性胰腺炎合并感染患者有效率高,能改善胃肠功能降低炎症反应并可缩短腹痛缓解时间及胃肠减压时间且不良反应少。
Objective To observe the curative effect and the effect of rhubarb combined with ustatin on gastrointestinal function in patients with severe acute pancreatitis complicated infection.Methods 85 cases of patients with severe acute pancreatitis combined with infection were divided into control group and observation group.Control group was treated with ustatin,treatment group was treated with rhubarb on the basis of control group.The total treatment efficiencies,the contents of plasma D-lactic acid,endotoxin(ET),diamine oxidase(DAO)and urinary lactofructose/mannitol(L/M)before and after treatment were compared between the two groups;the contents of serum tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and IL-8,the time of abdominal pain relief gastrointestinal decompression and hospitalization were compared between the two groups.The occurrences of adverse reactions during treatment of two groups were statistically analyzed.Results The total effective rate of observation was higher than control group(P<0.05);the plasma content of D-lactic acid,ET,DAO and urinary L/M were lower than those of control group(P<0.05);the levels of TNF-α,IL-6,IL-8 were lower than those of control group(P<0.05);the abdominal pain relief time,gastrointestinal decompression time and hospitalization time in observation group were shorter than those in control group(P<0.05);the incidence of adverse reactions during treatment was lower in observation group than that in control group(P<0.05).Conclusion Rhubarb combined with ustatin can improve gastrointestinal function,reduce inflammation,shorten abdominal pain relief time and gastrointestinal decompression time,and have fewer adverse reactions.
