摘要
目的观察比较奥氮平与氟哌啶醇治疗对精神分裂症患者阴性症状、执行功能及个人与社会表现量表(PSP)评分的影响。方法回顾性分析2017年6月—2020年6月经临床确诊的85例精神分裂症患者的临床资料。根据不同的治疗方法分为A组(n=44)和B组(n=41),A组给予奥氮平片治疗,B组给予氟哌啶醇片治疗。比较两组临床疗效、阳性与阴性症状量表(PANSS)评分、PSP评分、威斯康星卡片分类测验(WCST)评分、生活质量与满意程度评估问卷(Q-LES-Q)评分及不良反应发生情况。结果A组治疗后总有效率显著高于B组(P<0.01)。治疗后,两组PANSS评分均较治疗前明显下降,PSP和Q-LES-Q评分均较治疗前上升,且A组PANSS评分下降较B组更为显著,PSP和Q-LES-Q评分显著高于B组(P<0.05,P<0.01)。治疗后,两组持续错误数、随机错误数均较治疗前降低,完成分类数、总正确数较治疗前上升,且A组各项WCST评分变化较B组更为显著(P<0.05,P<0.01)。两组不良反应发生率比较差异无统计学意义(P>0.05)。结论奥氮平治疗精神分裂症患者的整体效果优于氟哌啶醇,奥氮平更有助于改善患者阴性症状、执行功能,提高其社会功能及生活质量。
Objective To observe and compare effects between Olanzapine and Haloperidol on negative symptoms,executive function and personal and social performance scale(PSP)scores in treatment of patients with schizophrenia.Methods Clinical data of 85 patients clinically diagnosed as having schizophrenia during June 2017 and June 2020 was retrospectively analyzed.They were divided into group A(n=44)and group B(n=41)according to different treatment methods.Group A was treated with Olanzapine tablets,while group B was treated with Haloperidol tablets.Clinical efficacy,positive and negative symptom scale(PANSS)scores,PSP score,Wisconsin card sorting test(WCST)score,quality of life enjoyment and satisfaction questionnaire(Q-LES-Q)score and incidence rate of adverse reactions were compared between two groups.Results The total effective rate in group A was significantly higher than that in group B after treatment(P<0.01).After treatment,PANSS scores were significantly lower,while PSP scores and Q-LES-Q scores were higher than those before treatment in two groups,and the decreased PANSS scores in group A was most significantly,the PSP score and Q-LES-Q score in group A were significantly higher than those of group B(P<0.05,P<0.01).After treatment,numbers of continuous errors and random errors were lower,while numbers of completed classifications and total correct numbers were significantly increased than those before treatment in two groups,and the changes in WCST scores in group A were more significant than those in group B(P<0.05,P<0.01).There was no significant difference in incidence rate of adverse reactions between two groups(P>0.05).Conclusion The overall effect of Olanzapine in treatment of patients with schizophrenia is superior to that by Haloperidol,Olanzapine is more helpful in improving patients'negative symptoms,executive function,their social function and quality of life.
