摘要
目的:探讨基于表面肌电图针刺联合体外冲击波治疗对脑卒中偏瘫下肢痉挛的干预效果。方法:选取我院收治的脑卒中偏瘫患者下肢痉挛患者80例,采用简单随机化原则分为两组,各40例。对照组给予体外冲击波治疗,治疗组在对照组基础上给予基于表面肌电图针刺治疗。采用MAS评分、MBI评分和FMA量表评分对两组患者进行疗效评定,对比两组患者表面肌电图(sEMG)检查情况。结果:经过治疗后,两组患者MAS评分明显降低(P<0.05),且治疗组MAS评分降低较明显(P<0.05);患者治疗后MBI评分显著升高(P<0.05),且治疗组升高较多(P<0.05);两组患者治疗后FMA评分显著升高(P<0.05),且治疗组升高较多(P<0.05);两组患者治疗后肱二头肌IEMG和股二头肌IEMG显著升高,膝屈曲CO明显降低(P<0.05),且治疗组改善较多(P<0.05)。结论:采用基于表面肌电图针刺联合体外冲击波治疗脑卒中偏瘫患者下肢痉挛具有较好的治疗效果,能够提高患者下肢功能,具有较好的临床推广价值。
Objective:To investigate the intervention effect of acupuncture based on surface electromyogram(sEMG)combined with extracorporeal shock wave(ESW)in the treatment of lower limb spasm in stroke patients with hemiplegia.Methods:80 cases of post-stroke hemiplegia were randomly divided into the control group and the treatment group,with 40 cases in each group.The control group was treated with ESW;on which basis,the treatment group was also treated with sEMG.The efficacy of the two groups was evaluated by the scores of MAS,MBI and FMA,and sEMG examination was compared between the two groups.Results:The scores of MAS were significantly decreased after the treatment in the two groups(P<0.05);the decreases in the treatment group were more significant(P<0.05).The scores of MBI and FMA were significantly increased after the treatment in the two groups(P<0.05);the increases in the treatment group were more significantly(P<0.05).In terms of sEMG,the biceps IEMG and biceps femoris IEMG were significantly increased,and the knee flexion CO was significantly decreased after the treatment in the two groups(P<0.05);of which the improvements in the treatment group were more significantly(P<0.05).Conclusion:Acupuncture based on sEMG combined with ESW has a good therapeutic effect on lower limb spasm in stroke patients with hemiplegia,and it can improve lower limb function.
