摘要
目的:探讨游离轻链与免疫固定电泳检测对多发性骨髓瘤的诊断价值。方法:选取2015年1月~2019年7月就诊于某院的45例多发性骨髓瘤患者为观察组,同期行健康体检的45例健康者为对照组,均行游离轻链与免疫固定电泳检测。记录观察组血清免疫固定电泳分型,并对比两组游离轻链结果。结果:45例患者中血清免疫固定电泳呈现不同类型的M蛋白区带,比例最高的为IgG型,其次为轻链型。λ型多发性骨髓瘤19例(42.22%)、κ型26例(57.78%)。λ型组游离轻链λ值、κ型组游离轻链κ值、游离轻链κ/λ高于对照组;λ型组游离轻链κ/λ低于对照组;λ型组游离轻链λ值高于κ型组,游离轻链κ值和游离轻链κ/λ低于κ型组,差异有统计学意义(P<0.05)。结论:游离轻链与免疫固定电泳检测在多发性骨髓瘤患者诊断、病情检测、疾病分型、指导治疗等有重要价值。
Objective:To explore the diagnostic value of free light chain and immunofixation electrophoresis detection in multiple myeloma.Methods:From January 2015 to July 2019,45 patients with multiple myeloma and 45 healthy people were selected and divided into the observation group and control group.All of them were tested by serum free light chain and immunofixation electrophoresis.The results of serum immunofixation electrophoresis in the observation group was obsrved and the free light chains in the two groups were compared.Results:In 45 patients with multiple myeloma,the serum immunofixation electrophoresis showed different types of M-protein bands,the highest proportion was IgG type,followed by light chain type;There were 19 patients with multiple myeloma typeλ(42.22%),and 26 patients with typeκ(57.78%).The free light chainλvalue of theλtype group,the free light chainκvalue ofκtype group,and the free light chainκ/λwere higher than those of the control group,and the free light chainκ/λof theλtype group was lower than that of the control group,theλvalue of the free light chain in theλtype group was higher than that of theκtype group,and the free light chainκvalue and free light chainκ/λwere lower than those of theκtype group(P<0.05).Conclusion:The detection of free light chain and immunofixation electrophoresis is of great value in diagnosis,condition detection,disease classification and guiding treatment of patients with multiple myeloma.
