摘要
目的分析北京市医药分开综合改革按床日付费对某精神病专科医院住院服务的影响,为医保部门健全和完善相关政策提供参考。方法采用自身同期对照方法对改革前后数据进行配对t检验并统计分析。结果改革控制了住院总费用的增长,降低了患者的经济负担,提高了住院服务效率,达到了改革预期效果。但也存在医保补偿不足问题,未来可能会对医保基金使用、医疗质量提高、医院及学科发展产生不利影响。结论建议医保相关部门根据疾病类别、并发症、年龄、性别等因素进一步修订完善精神疾病专科医院按床日付费的补偿标准。
Objective To analyze the impact of per-diem payment on the inpatient service in the comprehensive reform of separating medical treatment and drug sales in Beijing of a psychiatric hospital,and to provide a reference for the health insurance department to improve and related policies.Methods A paired t-test and statistical analysis were performed on the data before and after the reform using the self-concurrent method.Results The reform controlled the growth of total hospitalization expenses,reduced the economic burden of patients,improved the efficiency of hospitalization services,and achieved the expected results of the reform.However,there is a problem of insufficient medical insurance compensation,which may adversely affect the use of medical insurance funds,the improvement of medical quality,and the development of hospitals and disciplines in the future.Conclusion It is recommended that relevant medical insurance departments further revise and improve the per-diem payment compensation standards of mental illness hospitals based on factors such as disease type,complications,age,and gender.
